Severe anxiety can do weird (and bad) things to your body. Muscle spasms are a very common symptom and feature of anxiety. Anxiety can also have subtle effects not only on your diet, but also your metabolism. It can affect your digestion. The acid balance in your stomach and the fluid balance in your intestine can change to the point where you literally can no longer metabolize some foods, and they pass out of your body mostly undigested. Stress and anxiety produce cortisol, which can affect fat distribution and metabolism.
Go ahead and have neurological and metabolic tests if you need to for your peace of mind, but it may be time for you to see a therapist for gender issues, and to see a doctor who has a comprehensive understanding of how anxiety and stress affect the body.
