The amount of forehead setback that can be performed is dependent on how big the frontal sinus cavity is and the anterior border of the brain. You cannot set back the forehead back any further than the anterior border of the brain or posterior table of the frontal sinus. The best way to estimate this is with a preoperative CT scan. If you do not have a frontal sinus cavity (occurs in up to 15% of the population) then the maximum setback could be minimal and the 8mm desired setback may not be possible. My experience with working with Dr. Spiegel for 5 years was that he almost exclusively performed Type 3 forehead on every patient. Dr. Spiegel also accepts insurance coverage for FFS. Currently, mainly the East Coast health insurance plans cover FFS such as BCBS of MA, Harvard Pilgrim, Tufts. Most of the West coast plans do not cover FFS. In my practice, I strive to perform at Type 3 forehead on every patient unless they do not want maximal setback. The approximate maximal setback can be easily estimated with a presurgery CT scan of the Face which we do in the office during the initial consultation.
