My GCS With Dr. Schaff (Germany) – 5th of July 2017

Just to let you all know...

I'M VERY VERY HAPPY  ;D

First and foremost: I'm living in Germany, and thus get to enjoy the wonderous benefits of the German health-insurance system. In case that sounded sarcastic, you might have had the right impression. I mean, they pay up to 17-18k (!!) for GCS, but you do have to jump through a fair amount of hoops for it. Not all of them that nice.

In any case, last July they told me the hoops I had jumped through were high enough, and provided me with a letter saying they would pay up to 18k for my GCS! I then forwarded the letter to Dr. Schaff (more about that here, including details on the technique) and got placed on their waiting list. This specific list is only for those who do in fact send them either the full amount (20k) or those who get the approval letter from their health-insurance. If not, the waiting times exceed 3,5 years. So, lucky me, as I did have the money!

Last April, I payed the remaining 2500 Euros. Schaff asks 20k for the whole thing, so the last 2,5k is still your own responsibility. After some trouble with their administrative department, they finally offered me a date: I'm up for the 5th of July!

It looks like we will be taken care of at the same month. Based on your description in the other thread there is no need for hair removal prior surgery, right?

My Surgeon will be Dr. Schrögendorfer and he is using Dr. Schaff's method as far as I know and he told me that he suggest hair removal.

I think I will have to talk to him again.

Okay! With about 6 weeks left to wait, I'm starting to ponder about the things I need to arrange for. Here's a list of the things I've yet to get or to arrange:

To arrange for
- Find a gynocologist who can prescribe the famous donut-pillow (Already got a number, still gotta call)
- To ask my GP: Blood-tests, ECG, surgery-prep
- Last pre-op blood tests and appointment with my endocrinologist
- Train tickets to go to Munich
- Train tickets for the way home. I can only book those at the last moment, since the day of release will be determined there

To buy
- a couple of nice extra pyjamas and nightgowns (2 or 3)
- get myself a new hand-mirror... the last one broke after dropping it on the floor
- A pair of slippers
- LAN-cable (the hospital apparently hasn't got WiFi yet...)
- As I'm an audiophile and love to spend money on tech: perhaps new headphones :D

Administrative stuff to bring to the hospital
- The letter from the health-insurance, in which they state they're paying for the whole thing ... not to forget
- Passport, other necessary paperwork
- Money

Stuff I've already got, but which I should bring
- Bathrobe (apparently already provided by the hospital, but I like mine)
- A belt, on which I can hang the bladder-catheter and bag when walking around
- Clothing and underwear
- Books
- Laptop

Necessary stuff for the first weeks at home after the surgery
- Tons of toilet paper
- Plastic sheets to cover the matrass when dilating
- Hip-bath to wash myself before and after dilating (perhaps they gyno can get me a prescription for this as well)

With Dr. Schaff, the dilators are provided for by the hospital. At least that saves another 60 Euros or more.

I'm trying to document as much as possible about the SRS procedure at Dr. Schaff in Munich here for  international audiences since there isn't much info out there (1/3 of his clients come from abroad, mostly from the States and other European nations).

So... Basically all is ready now! Tomorrow morning in exactly one week I'll be leaving for Munich, for the greatest adventure of my life :)

How do I feel about it? Well I'm finally letting it in! (boah that took a while...).

Right now I'm still finishing the last little things here, and the last 3 weeks have been exceptionally busy, so I didn't have much time to think about anything at all. Oh, there's one very cool think I managed: to lose 2,5 kilos in just under three weeks :D --> this also means my BMI is at 25 now, so I've actually made my weight target. Couldn't be cooler, right?

As far as anxiety was concerned, it was a bumpy ride. Me, I tend to be a perfectionist. Everything I do, all has to be under control... Of course that's a pipedream when you lend your life to a couple of doctors, so that caused some panic down the road. However, since I spent most time at the gym, this actually really gave me a chance to relax and get back to reality.

Furthermore, my straight-edge policy has been working out really good. No herb, no tobacco (didn't smoke anywayz), no alcohol, no caffeine, no nothing. There were a couple of moments where I did try again, but the whole anxiety thing didn't work so well with that. My life has just been about going to bed early, eating superfood, relaxing as much as possible when having the time (long walks in the forest), and doing sports. Well last weekend there was an outdoor party in the forest to the north of my place, a friend went there and had a really nice dancing night until long after sunset. That actually made me a *bit* jealous at first. But then she called me because she was too sick to even stay at her place alone (big party slated for that night) so I had to come and rescue her. Seeing her with her hangover made me feel a bit good about myself again.. Well I felt pity for her feeling so bad, but at least I knew I had taken the right decisions for myself. So, I cooked her a lovely superfood-meal, we watched TV and had a blast. A straight-edge blast :P

Long story short: Here I am. One week to go, the clock is striking five to midnight and I've basically done all I could. There were a couple of mistakes made, but all in all, I'm here as fit as I can be and getting *really* ready for the big leap. This weekend a friend of mine is coming over from afar, she'll keep me company during these last few days before... before I get my new life :)

Oh well, here we go then :)

Basically I've been in the hospital in Munich for the last 7 days now, my surgery took place last wednesday around 12pm local time. The first week has actually been really good, I only needed pain killers the first 2 days, as long as I don't move too much of course. Which I try not to too much, because of the risk of swelling etc. They put me on anticoagulants 48 hours after surgery, and have been enjoying a nice bedtime since. In this time, I've finished one book, and am already working my way through number two.

As far as the results are concerned, there has been a little swelling in the inner side of my small labia (which is actually made out of the glans). It turned necrotic after 4 days or so, but Dr. Schaff told me this happens from time to time, when there's a bleeding and an accompanying hematome inside this sensitive tissue, necrosis can happen quite quicly. It only affects the outer half millimeter of the inner labia skin, so it's only a small area which it doesn't run deep. According to the docs, the end results will be minimall comprimized if at all. They're satisfied thus far, and I'm putting on an ointment with special enzymes to help my body get rid of it a tad quicker. Everything else looks just peachy, the inner labia aren't swollen that much anymore, and the outer ones only slightly.

So my body has been doing its job good! Which is what I hoped for too, because I've kept to a strict diet the 8 weeks before and have been doing sports like never before. For now, there's another issue though. When you get surgery here, they stuff your vagina full with a huge tapon, basically it's a condom made of gauze which gets stuffed with more gauze. This thing is really thick and is meant to keep the circumference of the vagina as large as it possibly can, so the body can start healing with the proper bloodflow and with everything in its final position. As with all other female hygenic products though, you can't leave these in for too long. Blood accumulates inside them, and in turn they become a nice breeding ground for all kinds of bacteria. To make sure you don't die within the 6 days the tampon stays inside you, the nurses stuff you chock full with intravenous antibiotics around 5 times a day, but even that doesn't stave off your body's reaction to the foreign body for too long. In the past, the tampon stayed inside for 5 days, but they've changed this to 6 because of better results with wound healing. In any my case though, I got a slight temperature raise yesterday (day 5), which has gone up a bit more today (38,3 Celsius). The docs aren't too worried though, they removed the tampon this afternoon around 4pm and are pretty sure that's the gist of it. Let's see!

Oh yes... I'm finally allowed to shower tomorrow morning! Which I'm sure is gonna be a whole bunch of fun, considering I've still got a hose runnig out from under there, into a pee-bag. Taking the whole situation into account, I'm not doing too bad at all. Also, my amount of female-only experiences has skyrocketed within the last week, such as:

- Bleeding out of my pussy, all over the bathroom floor (ups, forgot the maxipad)
- In accord, learning how to properly place and replace those same maxipads
- Having to use a mirror to see my genitals
- Having a gynacological exam (which is much like being at the dentist, only you can actually talk instead of just gargle)
- Tampons, tampons, tampons...

So then: another update....

I've actually touched my own clit tonight. For the first time, like... ever. It's working. Good! Well there was nothing erotic about it, and I probably shoudn't have probed around myself there, but what the f. I was careful and I wanted to know whether there would be any remote chance I might actually be able to enjoy having sex with my new anatomy. So... Examination gloves: check. Ointment/lube: check. And here we go! It's actually part of a routine that I practice, putting ointment between my minora. Might as well go a little bit deeper, I figured. And boing! :D

That's actually a huge relief! I've also taken my time to inspect the rest of my freshly new pussy, and I can image we might get along just fine, hihi. Oh and my vagina itself is healing fine, plus: it's deeper than the length of the big speculum :) Ah, and the temperature thing I mentioned above has resolved itself. Phfew!

As far as the rest goes... I'll just have to be careful a bit. Like, sitting is bad. Biking is worse, and let's please forget about thai-boxing for the next few months. However, considering the circumstances: I seem to be doing better than most here. I'm up and about all day, and as long as I lean back, I can sit without my donut pillow just fine. This newfound freedom is being used of course, as I am discovering the hospital grounds now. Armed with a book, it's a fine place to be!

Schaff told me we might start dilating soon, and the catheter through which is still pee is going to be pulled within the coming week. Oh, and I've actually been able to shower too, for the last 5 days or so. Yeesssss :D

Okay, I'm going home on monday after almost 3 weeks in the hospital, and have a few minutes to spare. Which means: it's finally time to write my in-depth review of my sugery in Munich, carried out by the fanastic doctors Schaff and Morath.

The Technique

As said before, the technique carried out in this hospital is one of a kind. For the vulva, Schaff and Morath mostly work along the lines of Suporn and Chettawutt, thus using penile skin for the labia minora and pudendal cleft, and using the inner part of the foreskin to create a sensitive clitorial hood. However, they use the (homogue) scrotal tissue for the labia majora, instead of penile skin (as Suporn does). The urethra is sewed into the front part of the vaginal wall, which for the other part consists out of a non-meshed, hair-free scrotal skin graft. This means Schaff and Morath do not perform a penile-inversion technique, what they offer is their own unique take on Suporn's method. Other than the aesthetics, another benefit with their technique not being PI, is that there's much less tension on the penile skin, thus benefiting bloodflow and reducing the need of complications. Also, there's no need to construct a mons veneris afterwards.

And it shows! The results they produce are spectacular, visually and functionally on-par with the top Thai surgeons and fully lubricating with arousal, because the urethra is included into the vaginal wall. They urethra has lots of glands in them, called Littré's glands. These normally prepare the urethra for the coming ejaculation, lubricating it. With the urethra being placed in the vaginal wall, it keeps the vagina naturally clean and lubricated, providing enough moisture or penetrative sex. On top of that, Schaff and Morath also keep the Cowper's glands, which exit into the vulva, as their homologues do in cis-women. This means full natural lubrication while aroused, both inside and out.

Schaff and Morath operate in two steps, they do this mainly to leave excess skin in the first go so they can allow for more swelling without damaging anything. The second step mostly revolves around your own personal wishes, it can involve a labiaplasty, clitoroplasty, FFS, BA and a host of other procedures. Everything having to do with the vagina is covered in the initial costs, you will have to pay for FFS and BA though.

The Administrative Part

Needless to say, Schaff and Morath get customers from all over the world. This also leads to a long waiting list. When you call their clinic, you're invited to come for a physical examination and a run-down of their technique and its associated risks. You might need to wait a couple of months for your appointment, depending on who you see exactly. I had my initial appointment with Dr. Morath and had to wait 5 weeks, but if you want to talk to Dr Schaff (who invented the technique) you'll have to wait up to 4 months. After the talk and examination, you get your surgery appointment, which will lie 3,5 or 4 years into the future. Not to worry though, there's another waiting list.

When you live inside Germany, you can get your german health-insurance to pay for most of the costs. Germans will have to pay an extra 3000 euro though. When germans send the written confirmation from the health-insurance, they get placed on another waiting list, which is only 1 year long. The same counts for international customers, if you pay the costs upfront, you get placed on this much shorter list. You will then get a call a few weeks in advance with your final surgery date.

The second surgical step has to be at least 6 months after the first one, and can be scheduled by phoning the clinic after the first step is done.

The Hospital Stay

Which brings us to the most important part: the hospital stay! You will be asked to check into the hospital in Munich 24 hours before your actual surgery, so you can go over the details one more time with the surgeons. Also, you get an appointment with the anaesthesiologist and get to drink a nice amount of disgusting water laced with a laxative. After that, you spend the rest of the day on the toilet. Fun guaranteed! :D

The next morning, you get to dress in a nice sugical gown and compression stockings. Half an hour before you're up, you get a benzodiazepine (lorazepam) to relax you a bit, after which you get wheeled into the operating theatre and the docs get to practice their magic. The surgery itself takes around 4 hours, so you can expect to be back in your room sometime in the evening. There will be a bladder catheter running out of your urethra into a pee-bag which you get to carry around all the time.

The first week, you have like a sponge sewed between your labia majora, this is to keep the area under a moderate pressure to enhance blood flow. Also you will have a huge tampon in your vagina, which also stays the first 6 days. This tampon serves to suck up blood and to have the vagina heal into its final position and configuration for as long as possible. In order to prevent swelling, catheters are placed into the vulva to let go of any excess exudate. As long as this sponge between the labia and the huge tampon are in place, you can't shower and walking also isn't that much fun.

The amount of care you get is phenomenal. Doctor Schaff or Morath will check on you every day for as long as you're in the hospital, keeping a close eye on your healing process and thus minimizing the risk of complications. On the 6th day, you get to get your first gynaecological survey! Yep, you get to walk over to an examination room with a gyn-chair, place your legs onto the pedestals and up you go :) This is when they pull the condom and remove the sponge. They will then clean your vagina with a saltwater solution, and drench a piece of bandage with wound-healing ointments (Furacine and Iruxole). This then gets inserted into your vagina again, and a thorough examination of the vulva follows. This ritual is then repeated every day. Taking out the bandage, cleansing the vagina, putting a new bandage in and inspecting the area.

The bladder catheter will be removed between the 9th and 14th day after surgery, depending on how well and fast you're healing. Two days after the catheter will be removed, your vagina will be thorougly looked at again and as soon as it has healed adequately, you can start dilating. As soon as you've managed this, you're released from the hospital. My hospital stay was 20 days in total for the first surgery.

During this time, the nurses will check on you 5 or 6 times per day, changing your bandages and providing you with ice packs for cooling. They provide you with antibiotics, painkillers, sleeping pills and whatever other pills you might need. Also, the hospital food is actually quite good, you get to choose between 3 menus, including a vegetarian one. The food isn't overly cooked and the cooks clearly like their job. For me, this was the first time hospital food actually made me feel like eating, hihi. Furthermore, as soon as you can walk around, you can go to the hospital gardens and make acquaintance with one of the cats who hang around there, or with the squirrel which gets so curious it might actually eat the food right out of your hand. If you sit still enough that is!

Conclusion

The technique, surgeons, hospital and staff are phenomenal. Contrary to many other great surgeons, you don't actually get discharged from the hospital after a week, having to spend the rest of your stay in a hotel room you will have to pay yourself. On the contrary, you get round-the-clock care from the surgeons themselves, other doctors and a great nursing staff. And the results of their works are fantastic!

I would recommend Dr. Schaff and Dr. Morath in a heartbeat, there aren't any better places to get this surgery done.
