HRT Providers by Location

The Wiki is in the process of creating a comprehensive list of transgender healthcare providers who prescribe, administer, and monitor HRT to better help people find affirming care in their area. This information is often buried in strange sections, mentioned haphazardly with no additional detail, or left off of provider websites all together. It would be a great reference for folks at any stage in their transition - pre-everything folks looking to start HRT, folks who are moving and needing to change providers, or maybe those who are just looking for new options.

If you are on HRT, we'd appreciate it if you could respond to this thread with the following information. You are also welcome to privately message any member of the Wiki staff if you feel uncomfortable posting this information publicly.
