I have been considering a forehead contour and I had XRAY, and it seems that i have a weird type iii forehead with enlarged frontal sinus.

This is only my observation but, is it right? Will it be much difficult to undergo surgery on me?
What are the risks of it? (type iii surgery)?

I am a male who wants to reduce his big brow bone.

Welcome to Susan's Place. I know very little about FFS because my knowledge is limited to what I have seen here but my opinion is that you would be better off seeking one of the big names in type III surgery. Yes, it will cost more but if I understand what I am seeing is correct, you would be seeking a revision if you settle for anything less than one of the best. I have seen sinus cavities similar to yours on the site but yours may be one of the most difficult to correct. From what I understand, the bone would have to be reshaped and moved to it's new location and simple shaving wouldn't do the job. Other more qualified members should be checking in soon with their opinion.

Whether or not to get forehead reshaping is up to you and your surgeon.

If you want to transition to female, I would definitely say to consider forehead reshaping. If there is no transition in your future, then it depends on your self image. Some men have very large brow bones and others don't. It is really up to you.

I had type III about four months ago and healing just takes time. The surgery is the same for both males and females, either trans or not. The risks are the same as for any plastic surgery on the head.

A consult with a qualified FFS surgeon will answer many of your concerns.

Dr. Barry Eppley in Indiana does this kind of surgery all the time, for both trans women and for men who just don't like their very prominent brow bones. He did my "type III" forehead surgery for me, and I am thrilled and had a great experience. You might look into him.

Intriguing.  I'd be interested in hearing more about your experience and the cost, if you would care to share. :)

You really should check out Dr. Eppley's site. He also does a lot of custom forehead implants, though I have no personal experience there. You really need a doctor to tell you how much bone can be removed.

Most of us who have or will go in for facial surgery have Type III foreheads.  Do not be concerned about that.  With the Type III surgery, they'll cut out the brow ridge and replace it with a prosthetic made either from exogenous materials or with pieces of the just-removed bone.

With a large brow ridge, implants will not change the gendering you receive.  It isn't just the shape of the forehead, but how much it protrudes over the eyes.  Implants can round out the forehead, but won't change how much the forehead itself sticks out.

What you look forward to has already come, but you do not recognize it.

Well I had found your suggestion of this doctor interesting as he would be as close as Chicago based surgeons. However a quick search for him found ALOT of patient complaints. So much as he went to suing a former patient to force her to take down even pictures of her face because she was showing bad things he did. Let's just say she was not the only one writing about him though.

Honestly, scared the crap out of me!

Thanks for sharing this information, Josie.  I am in Missouri and thought how nice it would be to have someone close, too, but I had not done any research on him. ;)

Dr. Eppley did some major repair work for me. I'd had FFS years ago with a well known surgeon, but it was not done well and I had serious problems with my frontal sinus. Also, the original doctor had failed to remove enough bone and basically only did the center of my forehead, leaving my orbital rims untouched. Anyway, Dr. Eppley had to go in, remove a bunch of bone filler, re-shave my entire forehead/orbital rims, and make me a new frontal bone using bone grafts from my skull. He also repaired as best he could the poorly done forehead incision. The difference before/after is night and day, and he corrected all of the functional issues and almost all of the cosmetic ones. I am very, very pleased with the work he did for me and will be forever grateful that he was willing to take on a difficult case like mine.
