What would be the best procedure?  Deep pheno  peel, dermabrasion, lazer resurfacing,
Would any of the above handle acne scars, large skin pores etc,  and would it be better to do before ffs or after?
Needless to say my skin looks hideous  without makeup  ....decades of sun and alcohol  abuse have taken their  toll :-\

Skin is the area of the body that Dermatologists specialize in. A consult with a Dermatologist would be the best place to start. Then another consult with a Plastic Surgeon experienced with FFS. See what they both say can be done about your condition. Dermatologists can do Dermabrasion and Laser. Chemical peels are dangerous and can lead to additional scarring. Be careful about that.

It is impossible to accurately diagnose over the internet, especially without closeup pictures.

If your situation is as severe as you said it is, you can forget the Med Spas that do things like RF skin tightening and such. I tried RF skin tightening after I lost over 100 pounds and it turned out to be a big waste of money. Med Spas are OK for ladies wanting to take a few years off their face by removing fine lines and tightening slightly loose skin.

To get real results for a severe condition, see a Dermatologist and a Surgeon who specializes in FFS.

I have large pores and plenty of blemishes etc and I was prescribed an ointment called Retrieve. Costs about $50 a tube and is a Retina A ointment. Its like having a constant mild peel...I have seen huge improvement in my skin tone and my pores have definitely gotten better. I was told that one of the laser techniques will take care of larger pores but I can't for the life of me remember what it was called. It seemed liked some kind of laser resurfacing. For me seeing a specialist is $$ well spent.

I've had good results from micro-needling at a clinic.  Sounds and seems a bit scary but worked great, the process is similar to aerating a lawn by punching rows of holes in it to trigger new growth and get rid of stuff you don't want.  The electric device looks a bit like a Dremel, sounded like an Norelco shaver but vibrated quite a bit.  Technician applied Emla numbing cream to my forehead (she said that's the most sensitive area due to thin skin), and an ointment to rest of face.  Process took a half hour, 3 sessions a month apart.  Pain level wasn't bad at all.  In some areas I found it ticklish and couldn't stop laughing... the technician said she had never had a client do that.  Cost $700 USD for the 3 sessions.  They said expect to stay home for a full day, I didn't need to - went back to the office next day, looked like I had been out in the sun a few minutes too long.  Within a week, pores were definitely smoother and some bumps and blemishes were gone.  After 3 treatments, definitely better looking skin.

I had considered a laser peel but had read about good and bad results - risk from incorrect settings, some people have had permanent damage from fractional laser resurfacing.  I think micro-needling is safer as it's a simple mechanical process.  The needles are extremely thin, punch through to a lower level of skin to trigger collagen growth.  I didn't have bad acne, but the technician mentioned many of her clients come in for that.

When I had this done, the clinic (cosmetic surgeon) recommended a vitamin E oil to help with skin health.  I've been applying it to my face once per day and that also helps - I can see the difference.

Also you probably know this but - drink lots of just plain water, your skin will thank you.
