Since you're a minor, you'll need to have a parent/guardian's permission to go forth with HRT. And also since you're a minor, you may be requested to go through a few therapy sessions before you're able to get a prescription.
There's a great place in North Raleigh (not sure how far that is from where you live) called Creedmoor Centre Endocrinology where I go, and the place is just wonderful. One of their specialties is gender transition. They operate on an informed consent model, so I'm not sure if you have to do therapy or not. I was actually quite disappointed to find out I had gone through three months of therapy only to be given an informed consent sheet that I could've gotten right away. Oh well!
CCE is typically VERY quick in wait time. I had my appointment three days after they initially called me to schedule. I walked out of the first appointment with a prescription for T, and they had me come back a week later to show me how to do my shot, and that was it. Now I'm a month and a week on T and things are going very smoothly.
As far as cost goes, I have insurance so my copay is a bit different, but on the website it's stated that the consultation will be $100 and the follow-up appointments will be $50.

http://www.ccendocrinology.com/patient-resources.htmlI should also mention that CCE uses a website called Patient Portal, it allows you to directly contact your doctors via online messaging and it also gives you the results of your blood tests on your dashboard. You can reschedule appointments if needed through Patient Portal, too! It's very handy.
Needless to say I've had wonderful experiences at CCE. There is one doctor and two physician's assistants, I'm not sure about how you're assigned to which doctor but I see Candy Stringer and she is wonderful.
