FFS - Dr Peter Raphael (Dallas) ?

Hi Ladies, I am new to this forum. I need some help in making decision. Anyone have FFS experience with Dr Peter Raphael in Dallas ? I had Breast Aug & Trachea shave with him and I am satisfied with this work. But I don't see lot about him doing FFS in reviews & search. His office started working with insurance very recently and mine is pre-authorized by insurance. However he is out of network and I would have to co-pay a lot. I am little worried about the results. I can do this only once and have to get it right :)

Another question:  I see lot of posts on Dr Spiegel accepting insurance. I had a skype consultation with him and he gave me a quote for FFS. His office does not send requisition to insurance and need approval letter from insurance. How do I get insurance to give the approval letter ? I have insurance from Anthem BCBS (TX). Do I need to call insurance -> Pre determination and give them the procedure codes/diagnostic codes myself ? I tried this for my breast augmentation/trachea shave and it went nowhere.

I have seen Dr Raphael's work first hand and it is very good.  A friend of mine got quite a bit done and she looks great.

His protege's Dulin and Harris both supposedly also do very good work and are in network.  I have a consult with them next month.
