I'm 8 months post op I had srs the penile inversion with skin graft but my depth and width is not good and has got worse it closed up I can only fit one small finger in at this point. Anyways does anyone know if there are other ways to do the revision? My doctor doesn't do colon sigmoid technique and was wondering if anyone knows about other ways like a different kind of skin graft? Are they able to use a graft from thighs or abdomen? I have heard about thigh abdominal grafts but not to often I was wondering if that's possible because my canal made from the penile skin is not enough.

Revision GCS is more common than most people think. I know of at least 2 women who have talked about revisions. One has had her revision and another is thinking about it.

Depth increases from colon surgery is the most common and there are several US and Thai surgeons who do this. Dr Salgado in Miami is one US surgeon who does colon GCS. A search on internet websites should tell you which Thai surgeons can do this procedure.

Another technique is to use peritoneal tissue from the abdomen. Dr Ting in NYC is known for doing this procedure.

A free graft can be sourced from any skin either full or partial thickness. This is the technique used by Dr Schrang in Wisconsin. Unfortunately, he is now retired and I do not know anyone else who uses this technique.

I've had more than one surgeon tell me that in roughly 20% of their MtF GCS patients require a revision.

I would advise anyone to avoid Salgado. I don't know of anyone who DIDN'T need a revision after him and many were afraid to go back to him for anything and decided to get a revision with another surgeon.

This technique looks very interesting. I'm still having the penile inversion method done on me in 4 weeks. I already lubricate down there where my body pretty much self-tucks, LOL.

The sigmoid colon surgery is the best solution. Others options may result in the same problem being repeated. The sigmoid colon surgery is much easier and safer from infection as a revision surgery fron penile inversion.. I have a couple of friends that had this done in Thailand very successfully.

Before offering any suggestions, I'd be wanting to know what investigation has been done as to why this has occurred. Did you follow your surgeons post-op dilation procedure? If yes and this caused issues, were they discussed with the surgeon? If no, why not? This is part of what the dilation regimen is intended to prevent.

I'd be wanting to know why it requires a revision, as, outside of failure to dilate, the other options are likely to cause similar issues for any further surgeries.

Thanks so much for everyone's replies all of them were helpful and had good points! To why this happened to my vagina? My guess is that my penis size wasn't big enough to make enough depth/width. I know for the inversion they may use scrotal skin graft which my doctor did do but I was still left with a small canal it seems. Even at 7 months post op I was unable to fit the big dilator inside the vagina which is the average penis size (I've seen many penises even bigger). I got to the point where I was so depressed because not only did I follow the doctors instructions such as dilation etc but I did extra. I would even dilate for hours and hours trying to open the canal it was what it was and I can feel it wasn't going to get any deeper when the dilator was fully in (which takes over a half an hour). At this point that's when I knew I would need a revision. I still am unable to have sex and when I do dilate it hurts at times and the dilator isn't even that big. I have consulted a few doctors for the colon revision but what sucks is the waiting times to get a consultation and a surgery date so now I have to wait months-year to fix this and it is depressing because it makes me feel like less of a woman since my vagina isn't like the rest of the girls I feel like I'm re starting the srs process all over again. I have only consulted three doctors in the US and it looks like I will have to wait a long time to fix this. I would be open to going to Thailand for this but I'm not sure if they take my insurance since I'm from US. My doctor who did my surgery doesn't do colon technique but we are still discussing other graft options but I think the colon technique is more promising for me. The three doctors I have consulted so far for a colon srs revision are Dr. Salgado, Dr. Schechter, and Dr. Ting. I can tell I would have to wait a long time to get surgery with any of them. I had my srs covered under insurance so I'm not sure if I'm qualified going to a Thai doctor because I don't know if they take insurance or even US insurance which I am looking into now but if anyone has feedback about that that would help as well.

I've always heard that you can increase width through dilation, but not depth. You mention width being the problem in that last post. Have you tried Suporns dynamic dilation technique? It's supposed to be good for that kind of thing.

It took me 10 months to fit the 34 mm diameter. It felt really tight to begin with. I suggest using the smaller diameter first and wiggle it in a circular way for a while before going to the next width. After getting the depth wiggle in a circular way and widen the entrance again. Dont be static with dilation, be active. after 8 mos its healed. You need to be more agressive. Remember scar tissue is not so pliable.
