to be prescribed low dosages of Estradiol and Spironolactone for a year, after which the doctor will look at dosages and discuss body changes to see whether any increases need to be made?

Also, 3 months into HRT the doctor has not ordered blood work to check T and E levels, her reasoning being that the two are currently competing.  I definitely want to remain healthy, but at the age of 45 father time is not kind to me!

Your thoughts?

I really can't say. I know my doctor through informed consent clinic (is that what it's called?) has been conservative with increasing my meds so far. I think it really does depend on labs and hormone levels though.

How long have you been on a low dose now? If you just started then maybe wait a month or 2 to see someone else for a second opinion. Or as soon as you need to.

I would call that a conservative approach and, in own my case... There was a level check around the 3 month mark....

I started HRT 4+ years ago at age 52... Do you have health concerns that might be prompting the low dose approach or do you just have a MD who is conservative by nature?

I did pills for about 4 weeks and then switched to injections which is still my preferred delivery method and I have had good results for an old lady and continue to see positive changes to this day...

Onward we go brave sister!!!

My doctor is very conservative.  She started me on a low dose, then checked my liver and kidney function after two weeks.  The protocol she follows is that any time there is an increase in spiro, she checks kidney function after two weeks.  I have had two increases in my spiro dose, so twice, I've had two two-week followups.  Going a year without followup seems excessive.

Same with E.  My first prescription was for one month, then a checkup.  Then three months and a checkup for levels, for a couple of cycles.  Finally, now, after nearly a year, my dose is steady, and she has given me a six-month prescription.  She is not going to consider letting me go unchecked for a year unless everything is stable over shorter periods of time.

I don't know what is normal in your area.  If you attend a support group, you might want to check with other members to see how their doctors manage their treatment.

Did she do any blood tests before you started hrt?

Thank you ladies, and friends! I've been on HRT for 3 months.  The endo did not check T or E levels before starting HRT, and still hasn't.  I know there was a negative review against the doctor for her not ever checking T levels. My nipples swell up more so than ever if I touch them, but there is really no nipple soreness.  Just very slight redistribution if fat in the face, and my butt has gotten softer and bigger. The endo said that after a year they would check E levels and talk about any possible increases in dosages.  Here in Lexington, KY there aren't many options.

Not normal. Not following any established guidelines or protocols. If you can't find another doctor, I'd suggest bringing in published cross sex hrt protocols.

Brooke, to be honest I am afraid of the endo, or rather her assistant (an RN) who seems so matter of fact about how they do things. Is there a good document that I can share with her?

Is there a chart available somewhere that shows recommended testosterone and various Estrogen levels? Since I am unable to find any other doctors in this area, I'm about to go (at least partially) the diy route,  The spironolactone being subscribed is the generally recommended initial dosage (not sharing numbers per rules), while the Estradiol is being prescribed at much lower than suggested initial levels.  That's where I plan to help the cause, and I'm not waiting a year to find out whether the Estradiol will be increased,

AnonyMs they did a test for my lipids and sugar levels.  Doctor says that she does not test for T levels, but will look at E levels later.

Brooke, bringing in some docs sounds like a good idea, but I have a feeling it may result in them saying "it is what it is", at which point I would say that I'm going the diy route until I can find another doctor. 

Maybe I'll just load up on spray-on Estrogen 😅😅😅😅

Kathy, Kentucky is a pretty backwards state.  Hell, you're discriminated against here if you're a guy and your belly doesn't look like you're expecting twins.

Thanks, Ashley!  That is the chart I was referring to in my post about possibly needing to Diy, but I wish the page would show some numbers on recommended T and E values, or how to calculate dosages when needing  levels are too high or oo low.  Maybe I'm missing something?

The following table contains normal feminine levels and as they are so varied, doctors would normally target menopause levels up to a max of 200 pg/ml. Blockers don't always result in a drop in testosterone however when they don't they can still prevent the effect of testosterone.

WPATH have guidelines, as do the Endocrine Society. Both of these would be a useful reference to the OP. I won't link them, due to them including specific info.

Either of these would be a better reference than some old guide of levels and a comment on target E2 level that are a little old..

My Endocrinologist put me on high doses right away.  He also did liver function tests as well as the usual metabolic panel. There is little justification for a full year of low dose HRT, if transition is in your future and you have no contraindicating medical problems. I have heard of low dose HRT for someone who is questioning their dysphoria and would like a trial of low dose HRT, just to see how it goes.

I agree with the previous posters who suggested to get a copy of the WPATH guidelines or guidelines from any other reputable source, such as WEBMD of National Institute of Health. Present these to your doctor and if they are still reluctant to give you a full dose, start looking for a new doctor. HRT can be prescribed by your primary care physician or even from Planned Parenthood under an informed consent protocol.
