I was looking at photos from Marci Bowers, one of the few surgeons to post photos, at least on the west coast, and the labia minora looks, if one can call it that, like what it is, namely an inversion technique. There are folds, wrinkles, and inward insertion of the shaft skin.

Is it not possible to get a realistic look? Is that a secondary vaginoplasty? Can Dr. Bowers or other doctor here on the west coast, although I prefer her, create a normal looking labia minora?

From my perspective, I think that should be possible by having the vaginal canal being separate from the construction of the labia minora.

Dr. Alter and other doctors seem to go out of their way to post no photos. Dr. Alter's few pics do not qualify and are ancient. It is hard to judge a surgeon without knowing a key part of the process.

Having actually seen hundreds of vaginas (worked as a nurse, inserting catheters, etc..)  really no two are exactly alike and there are tons of variations.  Nothing will ever be 'perfect', just close enough

It ís a little known secret that most trans women will need a secondary labiaplasty in order to have well defined labia.  This is usually done 6 to 12 months after the initial surgery.

You have a better chance of getting defined labia in a single stage surgery with the scrotal skin graft technique instead of penile inversion, but there's no guarantee.

Op, I would put my results with Heidi Wittenberg (I was her #33) well in the realm of appearance of natal females. My gyn who I think hadn't had any trans clients before me says I look fine to her, my GF agrees as does the woman who did my electrolysis -- I think she's seen more vaginas both cis and trans than anyone else I know.

I'm at 8 months post-op now and while a few things are still settling into place, I feel appearance is about final. I would consider a revision to address a bit of scarring at my clitoral hood, however it's clearly still changing for the better.

Echo, the frequency of a second procedure for labiaplasty, as I've understood it is more like 20%, certainly not a majority. All of the top 1 stage surgeons I know of are using the relatively new approach of using the harvested urethra as a lining for the inner labia. I'm really glad for that, I'm already partly self lubricating for sex play.

Personally I think there's a significant difference between surgeons, and some are better than others, but I think its best to judge for yourself. If you search for "A collection of links to photos of SRS results" you'll find some. Its not a huge amount, but better than nothing.
