How to report top surgery to work benefits office?

So I finally got a date for my top surgery in August and I'm super excited, but now I have to make all my arrangements to get the time off I need to recover. It seems like 6-8 weeks is the recovery time for a double incision. My job is almost entirely standing and carrying things over five pounds and my company does offer medical leave.

I'm just wondering if anyone has any insight to how I should report my surgery to my company. They have a very reasonable anti-discrimination policy that I could take them to task of I really want to if they give me trouble, but I would prefer to do this as painlessly as possible.

Should I just be up front about it or is there some way to phrase things that might make things a little smoother? I have no idea what to really expect.
