Hiya anyone out there maybe advise or shed a little opion on my current medication regime
Im 58 m to fem, been on Evorel 2 patches changed twice weekly, Also on a Decapeptyl injection once every 6 months
Im worried that my hair may become even thinner than it is at the top of my head so wondering should I be asking for Finesteride (Propecia) tablets alo or will this interfer with my current medications?

Any lihht anyone can shed on this quandry i have with myself

Dear Ms Maisie    I am going to give you a non-answer to your question, please let me explain.
None of us here can give you (nor should we) medical opinions that obviously need to come from your doctor.   Many of us have our own experiences with HRT and our transition details, but all of our bodies are unique and it is your doctor that after looking your blood test and examaniation results that can tell you what you need to know about your specific head hair question....   

BUT first things first.......

I am glad that you have become a member of Susan's Place and that you have shared your posting with other members here on the Forums.
I am thinking that you may lots more questions and concerns, this is the right place for you to be to find out what others have done that may have been in your circumstances.

First off, Welcome.

I was on Estradiol Valerate IM injections, Spironolactone, Finasteride, and Prometrium. I see you're basically on a testosterone suppressor and estrogen. I spoke with my endocrinologist on the topic and her advise was "even with low testosterone levels, it is possible to have high DHT levels." I was on Finasteride prior to starting Micronized Progesterone. Both are 5a reductse inhibitors. I prefer the Progesterone, as I get other benefits from it than just halting hair loss. However, Finasteride does help some if you're still thinning from DHT production. I would discuss it with your endocrinologist. There are people that have had negative effects from Finasteride. I found it fairly mild.

I will add to the discussion my experience, not as a recommendation but just as a data point. I had been on conventional transition doses of oral/sublingual estradiol and spiro for about nine months and was still losing a large amount of hair daily. I had lab tests done regularly and testosterone levels were regularly in the 30's and 40's ng/dL. I talked with my PCP and she agreed that Propecia would be worth a try. After about three months, the amount of daily hair loss had dropped dramatically. fwiw, I also started taking biotin supplements about a month before stating the finasteride.

I've had mixed input from medical professionals about the need for it. Aside from the green light from my PCP, one doctor said that if I'm taking estradiol, I shouldn't need the finasteride for hair loss. Another said that if I'm still producing testosterone (I am), then some will get converted to DHT and that will impact hair loss. I had yet a third doctor tell me the finasteride can help strengthen and enlarge shrunken hair follicles over time.

I can't comment on your medication regimen. I've never used patches, am not familiar with Decapeptyl or have had discussions with any medical professional about either. I did change my HT regimen about three months after starting the finasteride. I changed from oral/sublingual administration to IM. Like Harley, I added generic Prometrium about six months after but continued to take the finasteride.

I've been using finasteride (and biotin) for about 10 months now. Objectively, I can say that I'm still experiencing less daily hair loss. Further analysis is more subjective. My hair seems a little fuller. My wife swears that it is coming back in darker. There may be some regrowth, but ironically, if so, it seems to have happened on the temple that lost the least amount of hair.

That said, if I had it all to do over again, I'd still add the finasteride to my regimen.

The generic name for Decapeptyl is Triptorelin. This medication is a gonadotropin-releasing hormone agonist. It is used as an anti-androgen, much like Spironolactone. It works by causing stimulation of the pituitary, thus decreasing secretion of luteinizing hormone (LH) and follicle stimulating hormone (FSH). It is the decease in follicle stimulating hormone that is responsible for your hair loss. This type of hair loss is different from male pattern baldness caused by DHT. Finasteride works by decreasing your DHT levels.

It is true that Decapeptyl will reduce Testosterone blood levels and therefore make the DHT levels go down, but the decrease in in FSH may more than offset the reduction in DHT as far as hair loss is concerned.

This sounds complicated and it is very complicated. The effects of hormones on our bodies have many regulating pathways and our bodies usually do a good job of regulating our naturally produced hormones. When we take external medications that effect these regulating pathways we sometimes get unwanted side effects.

The answer to your question about adding Finasteride to your hormone regimen is that there are no listed drug interactions with your current medications. Rather than adding Finasteride, I suggest trying a different method of reducing your Testosterone blood levels. You may consider a different anti-androgen or even an orchiectomey. Also, many times just taking Estradiol, as you are doing in the Evorel patch, will reduce your natural Testosterone production. Discuss your options with your Endocrinologist.
