Got my approval for breast augmentation surgery!!! Nervous about stuff

So, another milestone is being set in my life. As the subject says, I'm getting new boobs!  I am so happy, and I know this is a huge step for me, but unlike voice and lower surgeries, this one feels different to me for some reason. I've found myself looking forward to the relief of having gorgeous breasts for many reasons,  but have also found that this surgery is very....noticeable. I feel like I am going from a girl to a woman overnight, and I am nervous more than the other surgeries. My confidence is at odds maybe. I feel more than ready, it's been a dream to achieve body and mind congruence, so I guess that I am wondering if anyone else has felt this way about a surgery before, or one that's upcoming?

Thank you,  Maid Marion. I'm really quite excited about getting my new boobs. It's been on my mind for some time now. I think that once the surgery is done and it becomes my new normal, I will relax a bit. It takes a lot of confidence being a woman, and even a bit more, being trans on top of that.

I'm a little confused about what you're asking but I'll answer it to the best of my abilities based on my own experience. 

Breast implant surgery pros: I love not wearing a stuffed bra.  People say my breasts look and feel great.  Surgery wasn't all that painful compared to other surgeries I've had.  My insurance paid for all of it.  My nipples are MORE sensitive than before surgery which has been a pleasant surprise.

Cons: My breasts don't look or feel as much like real breasts as I was hoping.  My breasts don't move or jiggle as much as I was hoping.  I wish they were larger.  My breasts are a bit more asymmetrical than I am satisfied with.  My breasts deform when I contract my pectoral muscles because the implants are placed under the muscle.

Overall I'm glad I got implants but I am seeking a revision. 

I have not had surgery on my throat and I have not had srs but I had breast augmentation a year ago.  I've had 10 surgeries, under general anesthesia and I would say that the breast augmentation has been the easiest recovery and the least painful surgery overall.  Honestly they're just putting two bags in your body above your ribcage.  It's a pretty simple procedure compared to srs or internal surgery which I've also had (inguinal hernia repair and anterior approach diskectomy).  I would say that all of my surgeries have been successful, I'm happy, healthy, and very active.

Expecting breast implants to be "gorgeous" depends on your opinion of what makes breasts gorgeous.  I am of the opinion that implants are never going to look or feel quite like real breasts.  I am a little disappointed that my breasts don't look better.  I was hoping that they would look more natural.  Without being too graphic I will say that my breasts get a lot of manual action.  I've gotten opinions from multiple people, who have a lot of experience with breasts, and they say that my breasts look great.  Also, my breasts don't move much when I walk and aren't as squishy as I would have liked.  Again, other people have said they feel great when touching them, which they like to do a lot.  A good friend of mine is a transsexual and got implants and we've compared bossoms extensively.  I think hers are great and she thinks mine are great.  I've squished hers and they're comparable to mine.  Wearing a pushup bra causes my breasts to bounce more when I walk which seems counter intuitive but it's what I have experienced.

Something people don't talk about much is that implants placed under the muscle "deform" when the chest muscles are contracted.  Implants placed over the muscle don't have this problem.  The reason implants are placed under the muscle is to reduce the obviousness that they are implants.  I have found this deformation of my breasts to be difficult to deal with during intimacy.  I never knew how often I activated my chest muscles.  When my chest muscles are contracting the deformation of my breasts is quite ugly in my opinion.  You can google "breast implants contract when flexing" to see it in action.  Luckily I did so before surgery so I knew what to expect.  A big thank you to the women who have posted videos showing this.

Another issue with implants is rippling.  If you google "breast implant rippling" you will find extensive information on the subject.  I honestly didn't notice any rippling until I stumbled upon this information while researching revision.  I have slight rippling at the bottom outer part of my breasts when I bend forward.  You can't see it but you can feel it.  It's pretty subtle.

When they perfect fat transplants I will have my implants taken out and use my own fat for breast volume.  The problem I have with fat transfer, also called fat grafting, is that it costs around $10,000.00 and the success rate isn't good enough for me.  Also, the breasts only get bigger by about one cup size via fat transfer procedure.

I started out as an AA cup which basically means almost no breast but I do have very good muscle tone.  I work out a lot.  I have 560 cc silicone gel implants placed under the muscle via an incision in my armpit done by Adam Tobias at Beth Israel Deaconess Medical Center in Boston MA.  I'm somewhere between a C cup and a double D depending on who makes the bra and who's opinion on measurements I follow.  I'm 5'8" at165 pounds and wear women's sizes medium to large.  Each implant is 560 cc.  Generally, when people discuss implants they are talking about the cc of each implant not the total combined cc of both breasts although I have seen youtubers mix this up and say things like "I have 1000cc implants" but they're talking about the total content of both breasts.  This is important because one 500 cc implant is a very different size than one 1000cc implant.

The other thing about how many cc you're getting per breast is that the same amount of cc can look very different on different body sizes.  A small framed woman with the same implants that I have would look like she has gigantic boobs.  Another thing to keep in mind is that the same size implant can look bigger or smaller depending on the profile.  High profile breasts may look bigger than a low profile breast of the same size.  Basically, a good surgeon will handle most of these details so there's no need to agonize over minutia.  I would suggest to tell the surgeon what you honestly want.  Each 200cc represents about an additional cup size.

I'm seeking a revision with Dr. Tobias, my consultation is tomorrow.  Depending on how that goes I will choose him or Dr. Revis in Fort Lauderdale who has quoted me $10,000.00 for the full cost of the revision (this price doesn't include accommodations or travel expenses).  Dr. Revis has LOTS of before and after photos including revisions he's done and really large implants.  I was looking at Dr. Pousti in Southern California but he goes through the nipple and I would prefer an underbust approach.  I want breasts that are more symmetrical and bigger.  I know, it's a bit insane to go bigger but I figure that my breasts don't look or feel as natural as I had wanted so I might as well go full on bimbo in an effort to squeeze as much sexy fun as I can out of life. 

This surgery is noticeable?  Yes.  Boobs get a lot of attention.  I went full time before getting boobs and have been thrilled because I don't have to wear a stuffed bra anymore.  I have been surprised at how people don't check out my breasts as much as I anticipated.  There's a whole world of boobage that was unknown to me, which is odd because I thought I knew almost all there was to know.  I have found that the way to get more attention is to show more cleavage.  This attention is not always welcome.  If I go out showing cleavage I usually make sure I have the ability to cover up if I decide I'm not in the mood for dudes to all but leap into my bra and 40 something women to sneer at me.  One thing I find interesting is that women 60 and up seem to get a kick out of my boobs.  They give me adorable smiles, smirks, and the occasional giggle if I show a lot of cleavage and I'm interpreting this behavior as positive.  I find that 40 something women really don't like to see much breast unless they have big ones themselves.  20 somethings are all over the place in terms of reactions to my breasts.  Girls in their teens tend to just look at my breasts silently.  I do find that getting breasts definitely makes me more passable, which I heard about but didn't make sense to me prior to experiencing it.  I assumed that a bra stuffed with those chicken cutlet things would have the same affect on pasability as implants but it's very different and I'm not entirely sure why.

What about breast implants affects your confidence?  Do you mean confidence in how you look or in the safety of the surgery or the cosmetic outcome?

I'm so sorry that this post has gotten so lengthy, I guess I'm detail oriented.  BEFORE you go there's one more thing.  It is generally advised that you perform breast massage on your implants, for the rest of your life, especially if the implant is placed under the muscle.  During surgery they create a pocket for the implant inside your body and if you don't do this "breast massage" the top section of the pocket might close up which is believed to be a contributing factor in capsular contracture.  The "breast massage" isn't actually a massage at all.  The way the nurse showed me was to pinch the bottom of the breast which hurt and wasn't all that effective.  I showed her how I do it and she said that my method was perfect.  I also showed Dr. Tobias and he said that my method was perfect.  What I do is place one hand on the other and cup the underside of one breast and then moosh the implant upwards without pinching or stretching my skin.  What I'm doing is using the implant to keep to top of the pocket from healing shut similar to using dilators to keep the vaginal cavity open.  In medical terms I'm keeping the superior and superior lateral and superior medial margins of the pocket in tact.
