I'm about midway through this great story.  It's about an F to M from the time he discovers his true gender as a child in the early 50s to 1980.  It's set in rural England, Suffolk County and follows Mary/Marty on his journey to manhood.  Tremain is a great writer;

"He steered them on.  They saw moorhens busy at the rivers edge and the first of the willow leaves falling in showers, like petals.  He thought, loving Sandra is like being the moon and trying to warm the sky"
