I found a website that offers a free website to help you keep track of your calories and and exercise.  You can sign up at calorie-count.com.  I have found in order to successfully lose weight, you need to do 3 things:

1. Keep track of what you eat in a log.
2. Plan your meals in advance as much as possible. Planning works much better than just counting.
3. Exercise at least 3 times a week with a regular schedule.

This website will help you do the first item and I think you can use it to do the second one too.  It has a database of many food items that you can search with known nutritional values.  I had used one where you had to pay a certain amount per week, but I was happy to find this free one and thought I would share it with everyone.  Also remember, if you are on HRT, you should be signing up as the sex for which you are transitioning to, because over time your metabolism adjusts to that.  For instance, I signed up as female.
