I still don't understand SUBLINGUAL?

I heard it is generally taking pills UNDER the tongue. I still don't understand what difference there is in taking oral(swallowing the pill) or taking it under the tongue. Will both not by the end end into the liver?

If the sublingual method doesn't pass through liver, then why for example somebody who needs to take Premarin(which is classified as dangerous to liver) not just put it under the tongue to dissolve?

We can not in good conscience condone the self administering of these medications. Not only may self medication be illegal, but HRT medications can cause serious health problems, and many have the potential for life-threatening side effects that can only be detected and prevented with proper medical supervision.
