In short, "Gender Identity Disorder" is a term designed to have a stigma attached to it. With intersex people there is the additional twist "Gender Identity Disorder Not Otherwise Specified" or "GIDNOS". Which is designed to imply that someone rejecting the sex they were surgically assigned to as a child, if born with ambiguous genitalia is somehow even more mentally "Disordered".

Would this apply to someone who was born with both male and female parts? But the female parts were sewn up because the male parts appeared more developed and the parents wanted a boy?

Yes, Virginia -- that's exactly what GIDNOS is for.
