Sorry this is a rough chop job, I cut images from the PDF then pasted it into word. Can't do much fancier than that from work. Let me know if you have probs with this or if I left and dosage references in (just was a table on page 5ish): It is about 3 meg so I appologise for people with slow connections. Oh, it is also a Docx file.
