Some procedures can be, some insurance companies are starting to pay for surgeries, but most do not.

Most therapy in some cases and endocrinologists and hormones are covered.  Hysto may or may not be covered.  Top surgery maybe but usually not, lower surgery usually not.

Hysto cost $10-15000 depending, Top surgery $5-9000 depending, Lower surgery $9-22000 depending on procedure.

Totally depends on the insurance policy you have.

Insurance is not the same across the board, it's not like a policy, is a policy and they are all the same.  Depends on who wrote it and for how much.
