Penile inversion v penile scrotum surgery for GRS

Hi there,

My surgeon does either Penile inversion or penile and scrotal graph surgery for gender reassignment. So my question is which is best. What is the deference aesthetically, sensate and depth. From images that I have seen the penile-scrotal graph GRS gives the most natural looking.

straight up inversion is an older tech best suited to those not circumcised and leaves the scrotal tissues for vulva creation. that plus scrotal graft leaves more tissue for external genitals (since they are too fragile for a graft from elsewhere).

a fringe benefit of scrotal graft is more sensitive external genitals (if penile skin is used for labia minora).

It was always my understanding that which technique was chosen depended to some extent on how much material was there and your original anatomy to start with.

If you do not have a lot of material then peno-scrotal is pretty much a given otherwise your depth may not be adequate. I'm not convinced that the choice of technique alone has much influence on the appearance of the end result. That I suspect has more to do with the surgeon.

I don't actually know which technique my original surgeon used but I would imagine it was peno-scrotal both from the position of smooth hairless, and slightly winkled but originally hair bearing skin. Also I didn't have much material to start with. Despite peno-scroal being used the original result was actually not that good cosmetically.

Although since it has been tidied up by a second surgeon the results are now excellent.
