Just a stretched libret. I have mine done 3 times in a upside down triangle along with many others. Be considering stretching mine out some. But that is freaking huge....thats almost 1 inch there.

I'd hope that was photoshopped.  That's pretty disgusting to me, but any sort of stretching isn't really appealing to me.  I keep hearing of the smell and that just makes me shudder.

That's only when they don't clean their jewelry.

This is because they are lazy and don't clean their stretches. I take the jewellery out my lobes before getting in the shower and massage them with jojoba oil and they never smell.
