Need help with name change in NY

I'm looking for advice from people who've done it in NY. I'm under the understanding that it's not difficult to do, but the "petition for change of name" form at the NY courts website (http://www.courts.state.ny.us/forms/namechange.shtml) is very confusing to me. Can someone basically walk me through how to fill it out?

I know this is something my therapist knows how to do, but I don't have an appointment with her for another few weeks and I want to get this under way.

Also, is it true that there's a fee of $210.00?? That's 3-4 electrology appointments!
