I believe what makes some one TS or TG is probably innate.  I don't consider it a birth defect.  It is just something some people are.  Calling it a birth defect plays into the hands of the Laura Schlesingers of the world.  No being born LGBT/T is more a kin to having brown eyes not blue.  Just part and parcel of the wide range of traits people can have.
