In writer/director Ramón Salazar's eye-popping comedy- musical- romance, the title "20 Centimeters," refers to the length of Marieta's (aka Adolfo) penis. Marieta (Mónica Cervera) is a narcoleptic, pre-op transsexual prostitute who is saving her money to get her 8.78-plus inches turned into a vagina, but various friends and lovers complicate her life, preventing her from realizing her dream.

Marieta lives in an apartment in Madrid with her best friend Tomás (Miguel O'Dogherty), a dwarf who runs a scalping scam. She works the streets at night, but because of her narcolepsy, never seems to turn enough tricks.
