If taking non-oral bio-identical estradiol, you can argue with your doctor/surgeon and state that you are just like a ciswoman, you take the same hormone and in the same way (secreted directly into your blood) so that if a ciswoman is allowed to get major surgery without being asked to remove her ovaries or take a drug to suppress hormone production, you should also be allowed. Several studies have shown that when estrogen is taken non-orally, coagulation is not significantly affected, risks don't significantly increase and can actually decrease at levels typically seen for transwomen. My own experience confirms this as clotting times remain normal despite levels of estradiol between 1,000-4,000 pg/ml on injectable estradiol.
It's important to discuss about these things with doctors if we are to improve treatment of transwomen. If we stay quiet and blindly follow, nothing will ever change. We shouldn't have to go through these difficult times if not justified. We deserve right treatment and to be heard.
