is it possible to have top surgery with out of referal from a therapist? just pay for it and have it done?

Anything is possible, but I highly doubt that a surgeon who would perform FtM chest reconstruction without a letter would be a surgeon you would want anywhere near your body.
Nero was the Forum Admin here at Susan's Place for several years up to the time of his death.
