I have bee on mtf hormones since autumn 10, but now want to go to a doctor that would help monitor my medication - I did have a test at one point. I thought the therapist I've began seeing could help, but she is not well trained in this area even though I am speaking to her about my need to transgender. Any ideas here?

You really need to find a proper Dr as soon as you can. Taking these drugs without medical supervision is really risky.

Since you are seeing a therapist, she should be able to give you a letter for the Dr.

How you actually find one depends upon where you live.
