Does anybody know of any reputable sites where you can personally make an order for blood work, and what types of test should be asked for specifically to measure hormone levels?

seems like it would be easier to hit up the local LabCorp or other similar place than it would to find someone to do a blood draw and then comply with the standards for sending blood through the carrier process...

I had my blood drawn at Labcorp yesterday. I went in with a doctors prescription and insurance and had to pay them $50. They showed me though that they were going to bill my insurance $650!!!! I almost fell over. I'm sure the price is inflated for the insurance company so you may want to check first. There is also a place around here called Any Lab Test Now and they have a .com site (franchise) and they will do lab tests without a doctor's note. I think they ran about $350.

If you are looking for a lab because you are self-administering the HRT you should probably want to know that it is a heck of a lot cheaper to just get a doctor's prescription for what you need. I can get my HRT for about $30/month as all the drugs are on the "discount lists".

I started off doing it myself but when I decided it was going to be a life long thing I decided to suck up my pride and just go see a doctor to get a scrip, tests, and advice. In the end no body really cared, not the HRT doc, my GP (who I came out to at my physical), or the pharmacist that fills my scripts.
