Answer 1: My own experience with insurance companies is that they like to decide what gender and sex I am based on what procedure I want. If I want something female related, they like to deny it on the grounds that I'm male. If I want something male related, they like to deny it on the grounds that I'm female. But I have always appealed denials, and so far, I've always won.
