Templates for bathroom "Care Letter?"

Hi all,
Can anyone point me to a site or sites that have good templates for a "care letter" my therapist can give me for bathrooms?

They are usually referred to as "carry letters" because you carry them around.
"Care letter" does make sense too because it states that you are under a doctor's care.

Such letters are usually written for two separate purposes:
  • As a "get out of jail free card" if you are challenged by some sort of security somewhere
  • As documentation for a request to change the gender marker on a US passport or driver's license (where allowed)

Page 9 & 10 of the US passport gender marker change memo gives one such example.

Usually a regular carry letter will say that the patient is currently undergoing care
and will be living their life in the appropriate gender.
