Post Op Complications

Hi all,

I'm 8 months post op and I have a complication that doesn't seem to be healing.  Below the clitoral hood and to the uretha, the area is always red and sore, and the clitoral hood is swollen.  My primary doctor suggested coming out to the boards to see if anyone has had a similar complication and to see what is possible to alleviate it.  Hope some of you have an answer to this.  It is painful and irritating.....

I would call your surgeon and ask if such a result is normal or ask what you can do to make it better.  It sure doesn't sound normal.

Is it at the vaginal opening below the urethra?  Is there granulation tissue?

I am no longer in contact with the surgeon for a variety of reasons.

The tissue is above the uretha and the granulation is right under the clitoris

Hmm, don't know why you would cease contact with your surgeon.  You were a patient of his/hers and you should be entitled to ask questions just like this.

Perhaps a silver nitrate treatment would be warranted.  I had some granulation of tissue in that same area at my one-month followup and now almost two weeks later I am not noticing any granulation tissue.

The surgeon and I are communicating through our attorneys at present if you want to consider that communication.  She has never been responsive nor has she ever provided proper aftercare on any level.  She's something akin to a used car salesman and a press hound.  I consider her something of a Jekyll and Hyde personality with a mix of something akin to chaos theory. 

I've had serious surgeries in my lifetime by true professionals.  She portrait herself to be such prior the actual surgery.  Unfortunately, after the surgery, she unable  and unwilling  to diagnosis or treat the complications that were occurring.  She refused to engage in any discussion with other physicians to figure out the complications and continually told me to go to my primary care for aftercare.

Ultimately, I had to find another physician to give me aftercare.  He has been excellent, but we can't seem to figure out what to do with this particular complication.

Anyway, I've done the silver nitrate treatments since Dec. It does pretty well for most of the granulated tissues.

The redness doesn't seem to be coming from any of the granulation.  It's as if the skin graft isn't completely healed. The clitoral hood is really red and irritated and has been since December.   Definitely one of the complications I'm not finding anywhere in the boards or the web.

I've been using mineral oil for dilation and unscented baby wipes for cleanup (boy do these make life easy for cleaning things and me!)

I should have mentioned my primary physician did a culture as well as a we treated for a bacterial infection which did nothing basically.

First thing, oil based lubes are not really a good idea.
Stick with either KY style jelly or glycerin-based lubes (Astroglide, etc).

Are you talking about the vestibule made out of the leftover urethra?
I think that there is a lot of variability with people.
That tissue can be very sensitive and prone to damage from friction with clothing.
It also can exude variable amount of mucous.

Hi Renate,

I thought mineral oil was best as it can't grow any bacteria at all in it where the others can.  I have astroglide and some other lubricants, but had been told and read quite a bit that they can eventually can allow bacteria to grow....

If you took a ruler and drew a line from the bottom of the clitoral hood straight down to the uretha, and widened the line about 1/4 " in total, that is the portion that is within vestibule itself.  It's only 1/4" wide at any point.  Sorry to answer sporadically on the post,.

Can I suggest you try using canestan ointment. You should carefully wash and dry. Then apply the ointment, making sure it has been asorbed by rubbing it gently.

If it is what I think it may be, tinea cruris, then it will disappear.

I'm sorry to say that tinea cruris is not uncommon. It is more associated with men than women, but women do get it, they just seem to suffer it more.

I may be wrong. But the worst that can happen is it won't clear up.

As was suggested by somebody else, it may be granulation tissue and can be treated with Silver Nitrate.

I had a LOT of this tissue and have had my family doctor administer it 2x so far ...a 3rd time next week. It is helping, thankfully.

Without getting into a long discussion of lubricants, try using a water based lubricant, available from Babeland, or another sex aids website. Babeland explains the advantage of different lubes. K-Y can become irritating, so something else may be best for you. Mineral oil can lead to infections over time.

You would be well advised to see a gynecologist who can give you a decent examination and determine the possible cause of your problem. Granulated tissue is extremely common and easy to treat with silver nitrate swabs.
