Hi Jessica

Your suggestion of an "Informed Consent List" is an excellent idea. given the recent discussion in other threads on this issue.  I would suggest that a Wikipedia entry on Susan's would be a more appropriate location to place this list, because the list can be updated or amended relatively easy.

This form refers to the use of testosterone by persons who wish to become more masculinized as part of a gender transitioning process.

You are being asked to initial the various statements on this form to indicate that the risks as well as the changes which may occur as a result of the use of testosterone have been explained to you and that you understand them. If you have any questions or concerns about the information below, we encourage you to take all the time you need to: ask questions, read, research , talk with clinic staff and think about these important aspects of your treatment.

______   ______ ___/___/___ I have been informed that masculinizing effects of testosterone may take several months to become noticeable, up to five years to be complete. Some of these changes will be permanent, including:


Hair loss, especially at my temples and crown of my head and, possibly, becoming completely bald
Beard and mustache growth
Deepening of my voice
Increased hair growth on my arms, legs, chest, back, and abdomen
Enlargement of my clitoris

These additional changes will not be permanent if I stop testosterone:

Decrease of fat in my breasts, buttocks and thighs
Increase of fat in my abdomen
More muscle development
More red blood cells in my blood
Behavioral changes, similar to those experienced at puberty, and increased sex drive
Acne, which may become severe and may cause permanent scarring if not treated


______   ______ ___/___/___ I understand that it is not known exactly what the effects of testosterone are on fertility. I have been informed that, if I stop taking testosterone, I may or may not be able to become pregnant in the future.


______   ______ ___/___/___ I understand that there are brain structures which are affected by testosterone and estrogen, and that current medical science does not understand these adequately. I understand that taking a hormone may have long-term effects on the functioning of my brain which are impossible to predict. These effects may be beneficial, damaging, or both.


______   ______ ___/___/___ I understand that everyone's body is different and that there is no way to predict what will be my response to hormones. There is a very complex interaction in each person between all the different hormones. I understand that the right dosage for me may not be the same as for someone else.


______   ______ ___/___/___ I will have physical examinations and blood tests periodically to make sure I am not having a bad reaction to the hormones. I understand this is required to continue testosterone therapy through this clinic.


______   ______ ___/___/___ I have been informed that using testosterone may increase my risk of developing diabetes in the future because of changes in my ovaries.


______   ______ ___/___/___ I understand that the endometrium (the lining of my uterus) is able to turn testosterone into estrogen and so increase my risk of cancer of the endometrium. I have been informed that not having my period for prolonged times may increases this risk. In order to reduce this risk, another hormone may be recommended to induce a menstrual period (shed the endometrium) several times a year.


______   ______ ___/___/___ I understand that through an interaction in the blood, my taking testosterone may actually increase the effectiveness of the estrogen in my body. The results of this are not known.


______   ______ ___/___/___ I have been informed that if my periods stop while I am taking testosterone I probably will not be able to become pregnant. I understand that testosterone should not be used to prevent pregnancy. Even if I have stopped having periods I should still use birth control (preferably barrier methods) if I am having sex where semen could enter my vagina or uterus.


______   ______ ___/___/___ I understand the effects of testosterone will not protect me from sexually transmitted diseases or from HIV.


______   ______ ___/___/___ I understand that the effects of testosterone will not protect me from cervical cancer or breast cancer. It is important to continue to be alert to the health care needs of my body. I understand that annual breast exams and monthly self-breast exams are recommended, even after chest reconstruction. My provider may also recommend periodic pap smears.


______   ______ ___/___/___ I understand that fatty tissue in my breasts is able to turn testosterone into estrogen, which may increase my risk of breast cancer in the future.


______   ______ ___/___/___ I have been informed that testosterone puts a stress on the liver which may lead to liver inflammation. I will be monitored for liver problems before starting testosterone and periodically during therapy


______   ______ ___/___/___ I have been informed that if I take testosterone my good cholesterol (HDL) will probably go down and my bad cholesterol (LDL) will probably go up. This will likely increase my risk of a heart attack or stroke in the future. The rates of risks for FTMs on testosterone are similar to the risks that are found in non-transgender men.


______   ______ ___/___/___ I understand that there are emotional changes I will likely experience as a result of testosterone therapy, and that clinic staff can assist me in finding resources to explore these changes.


______   ______ ___/___/___ I understand that once injected, if I have any adverse reactions to testosterone I must wait for them to wear off.


______   ______ ___/___/___ I agree to tell my medical provider about any non-clinic hormones, dietary supplements, herbs, recreational drugs or medications I might be taking. I understand that being honest with my provider is crucial to developing a trusting relationship. Sharing this information will help my provider to prevent potentially harmful interactions. I have been informed that clinic staff will continue to provide me with medical care, regardless of what information I share with them.


______   ______ ___/___/___ I agree to take hormones as prescribed and to inform my provider of any problems or dissatisfactions I may have with the treatment. I've been informed that if I take too much testosterone that my body may convert it into estrogen. This may slow or stop the desired effects of the hormone.

I agree that if clinic staff suspect I may have one of these conditions, I will be evaluated for it before the decision to start or continue testosterone therapy is made.

I have created a main wiki page for Informed Consent (https://www.susans.org/wiki/Informed_consent) or Click on the Wiki link at the top of each page, then select HRT on the main Wiki page and then of course select "Informed Consent".

I would question your second link  Dr. Marlowe's Weight Loss Institute (http://marlowesweightloss.com/), a weight loss clinic for Informed Consent especially in regards to HRT?

Go and have a look a make suggestions or edit the page for you new links.  The outline is there, a more thorough introduction to the topic is one of the things that can be improved along with how we address the links or the contact information.

I actually went to an appointment at Dr. Marlowe's with Melissa42013. He used to have a HRT link on his site but it's not active any more. He may be getting enough business without the links or possibly not taking new patients but Melissa still sees him.

When I initially created the page I typed in Infromed Consent, which gave me a new page and of course told me that no page existed. Which I thought at the time strange that this particular topic did not exist.  So I created the page with the states in.  I have merged the page I created with the Informed Consent page.

I have another appointment with him in October so unless something changed he is still doing it. I will ask him though. He started offering this service because he had a friend that had trouble finding help and medical schools do not spend any time on it. He has a number of other trans patients.

That is AWESOME Natasha! I know several people looking for a doctor in that general area. Please keep posting these resources everyone! If you are not sure on a resource, just let me know what you know and I will take it from there.

Just let them know that it can take forever to get in. I think my first appointment had to be booked 4-6 weeks in advance for the initial consultation, then another month for the appointment where you'll get something done. Part of it has to do with how much stuff she is involved in, but I can't help but wonder if part of it is like entering a Buddhist monastery; you have to prove your worth by (figuratively) standing outside the gate.

She's good though.

I may have an apportunity to go to kansas in two months. I'm dying for hormone replacement, I'm growing balder and more haggard by the day.

So does 'informed consent' mean that the therapist still has to get to know you and mooch at least $500 off you, or does it mean tbey throw letters at you as long as you aren't a lunatic?

It means you sign a paper acknowledging that you know the risks and are willing to take them and live with the consequences.  You are agreeing not to hold anyone else liable or responsible.  If it turns out great - then great!  If it turns out like ->-bleeped-<-, well, nobodies fault but mine.

It's an adult thing.

I'm hoping that the clinic in kansas isn't full of manure and malarky, I may have a direct ride there, and I'll have that ride in two months, which may be perfect if the clinic's waiting list is long. Does anyone know if clinics like this see out-of-state patients? I've had surgery out of state before, and it there was no residency problem, but it may be different for clinics and especially hormone clinics that may want to keep up with you.

And again, is there a process to informed consent or do they pretty much say "This is america, here's the papers." on the first visit?

I'm going to guess that they can tell you their process if you CALL them.  I would be surprised if they just took walk-ins. 

As far as residency, if the patient is footing the bill, then residency should not be an issue.  I know that I could go to any doctor I want, but if it is out of network (no matter the reason), then I pay out-of-network costs.  If I were on State subsidies, then the State gets to direct whether I need to be a resident in order to receive non-emergency services... That being said...I would FULLY expect you to commit to follow-up visits, and without that commitment, I would be surprised if they prescribed ANYTHING.

I never had to sign anything when I found my doc in the early 90's...we verbalized our understanding.  But society has grown even more litigious over the past few decades, not to mention the upswing in people who think HRT is the cure for all that ails them...so the docs are going to look to cover their respective asses.

Well, to be honest, I just need to get the consent forms and approval, and then I can go to whichever endocrinologist I well please, most likely. It's really the psychological/legal approval that I need, as long as I get tested and prescribed by a informed consent clinic, I can probably take my papers, tests, and dosages to a local endo when I return home. I live in TN and there's absolutely no non-RLE endo's around here, but if I'm already on hormone therapy, there's not really a reason for them to deny local care. It's really the innitial approval and diagnosis that's worth the long haul.

And to be honest, posting something a forum is free, calling 14 long-distance clinics probably is not.

I would honestly question whether one doctor is going to accept your agreement not to sue a different doctor as an acceptable clearance for treatment.  If I were having foot surgery, the surgeon wouldn't call up the gastroenterology surgeon who worked on my stomach to ask whether I'd signed a form understanding the risk of surgery.

All I'm saying is that I would hope to get my initial blood test results, hormone therapy plan and dosages, and all the regular tests at whatever clinic does informed consent. Then, when I'm already on hormone therapy, it seems a possibility that I can go to a local gender friendly endo (that does not do informed consent), and simply say "I'm already on hormone therapy and have done all my RLE and such, please use my information from my previous Endo so that I can be taken care of here." It's what someone would do if they simply moved across the country.

Oh, and back to providing information on the clinics, I called the clinic in Kansas twice, and it said the number did not work, or some mess. The website for the clinic looked straight out of 1997 as well, I'm not sure they're in business. They aren't listed on the SusanWiki yet, either. I'll be researching for Informed Consent clinics in the Kansas City area and will post if I find any.

Sharon Dian Lee, M.D., is Assistant Clinical Professor of Medicine at the University of Kansas and the founder and director of Southwest Boulevard Family Health Care. She holds the dual role of Community Electives Coordinator and HIV and Palliative Care Consultant at the University of Kansas Medical Center Department of Family Medicine. In 1989 she formed the Kansas City HIV Women's Coalition, as well as the Permanency Planning Project of Kansas City, Kan., which, since 1993, has sought to provide services and assistance to AIDS orphans. Dr. Lee also serves on the board of directors of the Black Health Care Coalition, which addresses the health and wellness needs of HIV-positive minorities in the Kansas City area.

Dr. Lee has been intimately involved in HIV/AIDS research and patient care since the early years of the epidemic in the U.S. She has served as medical director of a number of Kansas City-area HIV clinics over the past 20 years, and was a founding member of the Kansas City AIDS Research Consortium, which operated from 1988 to 1995. I was going to mention the Howard Brown Health Center in Chicago, IL,  in boystown, but it seems someone beat me to it. 

My ex-girlfriend went there for hormones and was very happy with the treatment she recieved, and I would have gone there for T if circumstances hadn't forced me to move away from Chicago.

 Allowed me to start HRT without a therapist's letter after signing an informed consent form.
 He's also a really cool doctor, very easy going and helpful.
 I'm not sure exactly how much experience he has with trans patients, or who he is willing to prescribe. I
am MtF. So far I've had no complications in my 5 months of being on HRT.
 While I didn't have a therapist's letter, I did see a therapist (on and off) for a year and he spoke to my
therapist before giving me the informed consent form. I don't know if you would be able to simply call
him up and ask for HRT, but it's worth a try.

One person said in 2007: 'Dr Tellier prescribes hormones without the need of a referral letter. He follows the liberal protocols
established by some US clinics, with the objective to avoid young Trans people self-medicating. You have to sign a discharge
and go through several blood tests, but usually you get your prescription within a month of the first visit. His usual dosage is
150mg Spironolactone and 7.5 mg Premarin, but he will prescribe other forms of oestrogen if you ask for it. You have to
return to the clinic every 3 months to have blood tests. He is very serious about health issues. My own HRT was delayed
because I had some liver issue he wanted to investigate. Unfortunately, his clinic is opened only to people 25 and younger ...
don't bother going there if you are older. I don't know if he will take patients outside of the province Québec.' [

I can't really recommend him highly enough. He and his staff are abnormally friendly and completely accepting of trans
people. He's had hundreds of trans clients already, so it's nothing new to them. When you first talk to them, regardless
if you look like a boy or a girl, they'll ask you what name you would like put on the file, and how you would like them to
address you, etc. At worst, I was treated indifferently by one of the clerical staff. I've had several bad doctors in my own
city, so I travel 2 hours to a different city to visit him.
He does NOT require you to have any letters from psychologists or psychiatrists. He does however advocate that you
should have someone to talk to during the process because he's genuinely concerned for your health, mental and
otherwise. He's also very receptive to hearing your input on how you want to deal with things.
The prescriptions he gives are a little bit different from 'standard'. But I know he'd be willing to give you a different
prescription if you were concerned.
The medications he prescribed me are:
 Estradiol
 Prometrium (progesterone - and a pretty high dose)
 Suprefact (Buserlin - a gonadotropin releasing hormone)
My only complaint is the Suprefact is given as a nasal spray. It just bothers me, but we discussed it and it's supposed to
work better with this delivery method.

My experience with him has been very good. He does not require you to see a therapist although he can refer you to one
who works out of the same facility. I was prescribed hormones on my 5th visit (approx 3 months) which is his protocol. He is
quite thorough in discussing all of the issues around transitioning, he knows how to go about getting your documentation
changed, he can refer you to a plastic surgeon (he did so for me), he has a nurse on staff who takes your blood as well. The
entire office and staff are very positive and supportive. He is also very familiar with hormone regimens at least for MTF. He is

I scheduled an appointment a month ago (First of Nov) with Dr. Marlowe. I informed the receptionist I had a letter which she said wasn't nesccesary. I had my appointment today, which I carried my letter with me any way. He made a copy of it for his records.

He asked how I had heard about him, which I told him I seen his web page earlier this spring. I told him I was concerned when I called to make an appointment that perhaps he wasn't accepting new patients because the webpage about treating transgendered people had been down for some time. He informed me he was in the process of try to have a his website updated.

I can't believe it's literally been almost three months since I've looked at this thread, and look at all the news! So many non-letter clinics to choose from! Especially that heaven-clinic in Kansas, sounds right down my alley - no letter, willing to help people in poverty, ect ect. This is such great news!

Sorry for grave-digging, but I though this was a great thread that popped up on the first page of Google for me when I was researching this exact topic and I thought it was great for some of who have no doubts that this is what they want.

My only question, is anyone aware of any in the NW Portland Metro Area?

Thank you so much for posting on informed consent. Just today I cancelled a second appointment with therapist. I knew it would be a waste of time & money. Previously in Pensacola it was not difficult at all to find physicians for HRT.

Any help or advise on any active informed consent clinic/physician nearby to lower Tennessee and north east Alabama would greatly be appreciated.

it still boggles my mind how there isn't a list of informed consent providers out there that is maintained consistently.

My doctor is Eric Meininger in St Paul, MN. He is on Catherine Mateos list but the number is wrong. Here is the number to make appointments:

He's a great doctor and Gillette is a very friendly, albeit very busy clinic. Today I called to make a follow up appointment and they told me he was booked till January. that's only for first time patients tho, I got my appointment scheduled for mid september. if anyone would like to know more about my experience feel free to PM me.

Catherines website is down but maybe I'll try to email her. The list is over a year old now.
BTW I was looking for a "site suggestions" forum section but does anyone know how to edit the wiki here? maybe I'll send more emails....

I also had an experience with another doctor... she didn't want to write me a prescription because I told her some things that made her think I wasn't mentally stable. But, I think she could be a good fit for some, and I got an appointment with her much sooner, within a few weeks. I can find her info if you want it. Dr Meininger is always booked at least a few months out AFAIK.

Dr James Farrow at the Tulane University Uptown Student Health Center in New Orleans, Louisiana does hrt via informed consent.  He ONLY does it for Tulane students.

(There are others but these two deal primarily with trans* patients)

They do informed consent HRT as well as family medicine (primary care). They prefer to integrate your HRT with primary care as this allows them to fully manage your healthcare. I have gone this route and it is excellent. They take most insurances so visits and labs are covered. Phlebotomist is on premises so there's no need to go elsewhere to get your blood work.
