Title: Where should I start, as far as transition goes?

So, after a billion years of put-offs, <not allowed>-ups and general denial I want to start HRT. The only problem is...I have no idea how to go about it! Believe me, I've done so much research it feels like I'm back in public school in again, but I can't figure out the entire process.
I know I have to go to a gender therapist (if that's the correct term), but when? Should I get a therapist /before/ I attempt to get my blood work to check my levels, or should I just go to the doctor, tell him what's up and ask what his requirements are? About how long did it take for you to get your "informed consent" letter? I'm in no rush to start, but waiting a kajillion years for a piece of paper saying that I can "start" my transition isn't what I had in mind either.
Any comments, or suggestions? I'm flapping in the breeze here. xD

you need to find a therapist that deals with trans people and tell him/her that you want a letter for HRT then they can recommend you to an endo.

There is no universal law that says you have to see a therapist before being allowed to begin HRT, only guidelines. It also depends on which country you live in and on the individual requirements of doctors and clinics. Some doctors are willing to prescribe hormones without a letter and instead assess you themselves, needing only your informed consent.
