Title: Top surgery letter

The therapist who wrote my T letter is now willing to write my top surgery letter. I'll see her in a couple days and she told me to bring all letter requirements. I know specific surgeons desire different things in their letter. But I am just looking for general guidelines, or maybe even there's a template out there that we can work with. If anyone can point me in that direction or share some of the things their own letters said, that would be awesome.
