I just found this website that has kits on how to change your name and gender marker. Thought I'd post this for anyone in Mass who is planning on doing these changes and hasn't found this site.
From the main page, just hover your mouse over "Publications" and you'll see the different pages for the name change and then the gender marker change pages.
