Title: I hear air is coming out when I pee,
Post by: Snöfrost on April 06, 2015, 05:26:30 AM

I am currently 2 months post-op and I have noticed since 3 days ago that I can hear that air is being pushed out when I sit down and pee. I don't know if the air is coming out from the urinary tract or vagina. I don't have any pain that I am concerned about, but I think other kind of "pain" could be easily "hidden" by post-op pain.
However. I don't have any fever, haven't noticed any feces and it doesn't hurt when I pee. I will go to a doctor as soon they can give me an appointment, but right now everything is closed due to easter.
What else should I be aware of and what can I do myself?

well first of course, if you are worried I would encourage you to consult a fully qualified doctor. I am not yet qualified, and hence I cannot give you formal medical advice.
As a general observation however I would observe that your bladder is right in front of your vagina - and thus as you tense your detrusor muscle to void, you will also squeeze your vaginal canal - and hence any air pocket in there will be be expelled. Indeed I have experienced this myself, and I know some natal women also do.
Now as I explained I'm not yet qualified to tell you whether or not this is what is happening with you, nor indeed could I do so with any certainty without examining you and taking more details, but it sounds to me at this stage as though this may be a possible cause. So I repeat, if you are worried, go see your physician, which all anyone not medically qualified can (or indeed should) tell you.

I forgot to mention that the amount of air that is coming out is alot. It takes about 5 seconds until the sound of air stops. And that seems to be quite alot of air. And I don't know if my vagina can hold such amount of air...

This information does not change the differentials involved as actually you'd be surprised what is possible, but it does reinforce the only advice I can give which is to go see your physician as you are quite clearly concerned.

Sorry, I don't have any info or advice on this and you've said you'll see a doc and that's the best course of action. Hope it works out OK - please let us know.

Women have a hissing sound when they pee. I noticed that when I started using ladies' rooms and now notice it myself when I have a strong stream.
Might this be what you're hearing?

^^^^This. The sound could be described as a whistling under the right conditions for sure.

Medical advice is always good, but I'm also telling you, at 2 months post op, the swelling is still going down (it takes 6-12 months for it all to be "normal"). So there might well be changes in the shape of the urethral opening that make it "whistle" more now than it did two weeks ago. Which might go away in another 2 weeks, or stay there forever. There isn't really a mechanism I can think of logically that would allow a stream of air to rush out whistling as you pee... how is the air getting in? You'd have other symptoms if there were fistulas or something awful like that, such as fever and infection. A lot of ciswomen whistle when they pee. It sounds totally different than it used to, at least for me.

I am 6 months post op and there are times that I can feel that air is coming out from my vagina (like when I release gas) but I think this is just because of the pressure inside / air trapped inside sometimes when you move. I always smell my dilator after dilating but I never sense any fecal smell or check my vaginal canal when I poop but I never see any fecal going out from there so I am pretty sure its not a leakage or something. I also go to my SRS doctor monthly and he said he havent seen any issues inside my vagina.

I made research and was stuck that it could be a fistula. I called to the hospital last week regarding this and told them the issue. They were worried and they wanted me back ASAP. So I booked a flight in a real hurry to Karolonska Solna in Stockholm 2 days after the phone call to meet with one of the SRS surgeons. My ordinary surgeon wasn't available at that time. And when he looked, it looked fine and he was feeling with the fingers inside the vagina and the rectum. He couldn't feel any fistula. They also stated that they would have notice that there was a fistula while I was recovering on the hospital while I was recovering from SRS.
Anyway...the surgeon didn't know the cause. Before the meeting, I actually recorded the sound on film on my phone. He hasn't seen or heard anything like this. So he suggested that he would arrange an X-ray where I live to confirm what it could be.
And now the air can keep on to come out for 2 min, even if my bladder is empty.

I had a colovesical fistula a few years back caused by diverticulitis. My diseased colon fused to my bladder and leaked gas into it through the fistula. I was literally farting out of my urethra every time I peed. I also had the mother of all urinary tract infections.
This might not be the case with you, but fistulas are no laughing matter and need attention before the infections get serious.

I have a hissing noise when I pee sometimes more when I'm desperate so more pressure on talking to my friends it's pretty normal. I also have air coming out of my vagina this was put down to air getting trapped in there happens more if I've dilated less if I havnt.

Ps Ive got some good girly friends who I have been able to ask questions.
