(Not stating what my dosages are, just results, hope that's okay)
From the ballpark figures at this site:
http://www.hemingways.org/GIDinfo/hrt_ref.htm
It seems like perhaps I should want to keep my estradiol level where it is (317) for the sake of good development etc but my endo wants to lower my dosage by 25%
I am on transdermal estrogen and oral spiro only. ~6months in.
SPECIFICS:
Estradiol 317 H pg/mL
Estradiol, Free 4.42 H pg/mL
ESTROGENS, TOTAL 713.4 H pg/mL
TESTOSTERONE, TOTAL 8 L ng/dL

I would explain how you feel and why you want to remain at the current levels. I would also ask why they feel the need to lower your dosage. Mine lowered mine to keep me to try and keep me around 200. Hugs

Isn't long term exposure to estrogen (or testosterone) that works the magic and not the level? Just curious.

There is no IDEAL level for everyone. We all vary in our response, sensitivity. My levels are around 1,000-4,000 pg/ml.
The more important question to ask is do you feel good? is feminization progressing well? Higher levels aren't necessarily associated with increased health risks, some doctors ignore this. Risk is associated more with the type of estrogen and route of administration. I take bio-identical estradiol non-orally, as you are. My blood test results are fine. Studies have shown transdermal estradiol to be safe in men with prostate cancer (49-91 yrs old) despite levels being higher than 500 pg/ml.

You want good feminization, they want to cover their butt (imo :) ) .
You might say that with topical intake there is a very low risk.
Its different with oral intake. Even there sublingual intake can decrease risk greatly.
Some informed consent clinics don't regard levels much.
"Treat the patient not the lab."
Important is feminization and how you feel.
Pregnant women have much higher levels .
hugs
