4 months after GRS, I still cannot masturbate or orgasm and I think that I figured what was the biggest hurdle. Clitoris-wise, things have had a steady progression, I went from numb, to hypersensitive, to not hypersensitive but uncomfortable feeling. So even though I'm not there yet, I feel like the clitoris is slowly healing properly. There's one issue that hasn't improved at all since GRS though and it's pain on arousal.

When I get very aroused (looking at porn or having my gf touch me slightly), I get the feeling of having an erection and it hurts like hell. It's some pretty sharp pain so whatever happens next, my mood is ruined and I feel like crying due to the pain. I'm concerned that this could be due to leftover erectile tissue but visually it does not look swollen and my vagina does not close up. Is this pain normal? Why is it not getting progressively less painful?

It might be noteworthy to mention that before GRS, with androcur and no testosterone, I would still have plenty of erections from minimal sexual arousal. I feel like that might be part of the problem, but I don't know.

Perhaps some remaining stitches or granulation?

Did you have painful erections before surgery? If yes where was the painful area? Shaft or inside erectile tissue?

Also you shouldn't worry about it too much this early, it could just need time.

All stitches were gone at the second month and I do have granulation but inside the vagina.

Pre-op, erections were a little painful if I didn't masturbate in a long time, it wasn't painful like now though and it wouldn't hurt anymore on the next erection. Post-op, I tried enduring the pain of arousal and trying again the next day or later in the day but it doesn't get better as opposed to pre-op.

I have no idea how to describe where the pain was/is.

I am not sure about the details of your surgery but I suspect it's along the same line as mine. The Glans of the Penis was used to construct the clit. The glans was and is still able to expand in times of sexual excitement giving you that feeling of an erection. Mine was left intact making an oversized bump but from what I understand, some procedures reduce the size to make a more realistic clit. If your was reduced in size, some of the pain you are feeling could still be due the healing taking place. I would suggest you contact your doctors and learn more about the procedure and possible healing rate.

One recent Dr. Chettawut alumnae recently told me that he prescribed 'penis pills' (Tolperisone) to her.  They are intended to prevent errant erections of the healing neo-clitoris - in other words, minimise phantom feelings of the absent anatomy.

I hard fairly painful clit, it was more noticeable a month or so after srs, like it was erect for days and aching because of it. it was more painful than the srs itself, it doesn't happen anymore, it slowly went away. It was a really frustrating pain but i wouldn't have called it sharp. but I guess it was something different than your situation leviathans.

When I'm super turned on I get a tight feeling (which makes considering the mechanics of it), no sharp pain but I know the moment you are meaning. Probably best to ask surgeon about it

Hello everyone,  my clitoris gets fairly uncomfortable (swollen?) when aroused with no outlet for release.  It does go away fairly quickly after having sex.  I had toyed with an inexpressive battery operated vibrator in the past.  Found it worked to get me past the arousal state.  So, hoping for even better performance.  I picked up the a the 'Original Hitachi Magic Wand' at a local sex shop.  We are talking high power, plug in, big girl stuff! 

Wow!, it worked very well and feels pretty good.  No, thank you G-d orgasmic feeling, but good. 
I go easy around the clitoris and my arousal appears to be manageable as long as I can locate a power outlet.  The device is fairly large so not sure, no, wait for it, NO!, I will not lug the device around.  Oh I do miss my vibrating Skypager :(

Responses here are all over the place. This scares me as I hoped that this was just a ordinary problem but it seems like it's not. I still have to wait another month and a half before I can meet Dr. Bélanger... It's been 5 months since I had sexual pleasure, I wonder if I will get to the one year mark.

I went to Brassard and Bélanger, it's supposed to be made smaller but who knows? Maybe expansion is the problem, but I'm not sure.

I'll have to verify for sure but I doubt I have much testosterone left. Even on androcur, I had near zero testosterone and still had relatively high libido like now and easy erections.

The clit is still sensitive but it's not too extreme anymore, I don't think that the issue come from there because even when the clit was numb before, it would hurt a lot when I would get an "erection".

It's nice that could get pleasure from a vibrator but I can't. The hitachi just is too strong and a cheap bullet vibe on the lowest setting is still pretty uncomfortable. I still don't understand why they don't give instructions, they expect you to figure it out. Well, I can't figure it out, it does not feel good, wherever I put a vibrator on my body. And then, my mood is already ruined before I start due to the pain from having an "erection".

I was curious and asked my endo about this, and he mentioned that there's more than one androgen and the standard test for T may not pick it up, or something like that. In which case it your T could be the cause and it would still read very low.

I always wondered bout this i always thought what if you can I guess still get a hard on while your you know what is inverted inwards?!?! I do not know the details of this operation I rather not because well it will make it harder for me to go thru with it. Mind you I am the kind of girl that does not even like touching her you know what ..

Hello, it's more like swelling or at times like a blood rush to the clitoris.  Can occur without sexual arousal.
I have studied my pubic area during these occurrences and didn't notice any physical change to size of clitoris.

The use of my recently acquired vibrator appears to be working well at reducing residual swelling post-SRS.
As a result I now feel a pleasant sensation from touching myself.  Much more welcoming of light touching by clothing or sexual partners.
FYI: My SRS surgeon has recommended for months the use of a vibrator to get me to the BIG O.  Now a bit more pain free, pretty sure I'm close...if I let myself go there...kinda scared of how it will feel.

Update :

I saw a doctor at a clinic and I have a vaginal infection and had it for a while now. Hopefully taking care of this will help, although I'm not sure that this could be the cause for pain on arousal but we'll see.

Btw, the doctor called me a man while having his fingers in me for the vaginal exam. It was so freaking absurd. I pass as a woman but just because I was honest and mentioned having had GRS 4 months ago, he had to misgender me the whole time. I got really mad. At least after he apologized later and told me he was super impressed with the results, he never though it could look that good. He told me he would never have known I was trans from looking at my 4months post-op vagina. So at least I have a very good looking non-functional vagina lol.

I had a UTI and the doctor was getting the gendering right until he walked out the door for a while and then returned. I couldn't protest because I had just had voice surgery and I was still in the no speaking time but he corrected himself almost as soon as he said it. I think the best solution would be to say "excuse me" or "where" as a gentile reminder.
I didn't have the option of not telling him because the plumbing differences require a longer treatment with antibiotics.
