Title: Dr. Dugi (OHSU) update

In May 2016, Dr. Dugi performed his first vaginoplasty (GRS) at Oregon Health & Science University (OHSU). As of the end of August he has performed approximately 12 cases and continues to do 1-2 cases per week. I have heard from Dr. Dugi that his initial cases have gone well, but he has not yet shared details. Nothing has been published yet, but of course his first case just reached three months post-op.

OHSU accepts the Family Care option from the Oregon Health Plan.

Hello.

Dr. Daniel Dugi offers the vaginoplasty surgery which to set up a consultation will require a referral from your primary care provider and at least one letter of support from a mental health provider following the guidelines of the WPATH Standards of Care.  The essential content of the therapist letters is pasted below.

Once the referral and at least one letter of support is received Dr. Dugi's office will process the referral and send it to Dr. Dugi for medical review.  The clinic will determine if your insurance requires a prior authorization for the consultation visit.  Once approved you would be contacted by his office to schedule the consultation.  Currently new patient consultations are being scheduled in December.

Referrals and therapist letters can be faxed in to Dr. Dugi's attention at 503-346-1501.  I recommend you keep a copy of all of your letters and have them on file.

Dr. Dugi has a recorded lecture about the vaginoplasty surgery that I recommend viewing before your consultation appointment as this would give you information about his background, training, surgical technique, and information about preparing for surgery and what to expect for recovery.  You can access this video here.

Dr. Jens Berli offers both FFS and breast augmentation in the Division of Plastic and Reconstructive Surgery.  I don't believe the Washington Medicaid plan has created any exclusions for these services and each referral would need to be reviewed for authorization through the Washington Health Authority's Transgender Health Program.  I recommend you still use the PCP referral process for FFS and breast augmentation.

Referrals and therapist letters can be faxed in to Dr. Berli's attention at 503-494-1717.

WPATH letters of support should include:

Regarding the client:

·         The client's general identifying characteristics;

·         Results of the client's psychosocial assessment, including any diagnoses;

·         The duration of the mental health professional's relationship with the client, including the type of evaluation and therapy or counseling to date;

·         An explanation that the criteria for surgery have been met, and a brief description of the clinical rationale for supporting the patient's request for surgery;

·         A statement about the fact that informed consent has been obtained from the patient;

·         A statement that the mental health professional is available for coordination of care and welcomes a phone call to establish this.


Regarding that you as a clinician have:

·         A master's degree or its equivalent in a clinical behavioral science field. This degree, or a more advanced one, should be granted by an institution accredited by the appropriate national or regional accrediting board. The mental health professional should have documented credentials from a relevant licensing board or equivalent for that country.

·         Competence in using the Diagnostic Statistical Manual of Mental Disorders and/or the International Classification of Diseases for diagnostic purposes.

·         Ability to recognize and diagnose coexisting mental health concerns and to distinguish these from gender dysphoria.

·         Documented supervised training and competence in psychotherapy or counseling.

·         Knowledgeable about gender-nonconforming identities and expressions, and the assessment and treatment of gender dysphoria.

·         Continuing education in the assessment and treatment of gender dysphoria. This may include attending relevant professional meetings, workshops, or seminars; obtaining supervision from a mental health professional with relevant experience; or participating in research related to gender nonconformity and gender dysphoria.
