Title: Morning sickness on estrogen?

Hi folks,

About six weeks ago my Gender Identity Clinic raised my estrogen dosage to 'therapeutic levels.' I can't put the figure but I'd consider it to be relatively high for someone who has no testosterone in the system (I've had a bilateral orchiectomy).

Since then, at least I think it's since then I've noticeably been feeling nauseous. I suggested today to a friend that perhaps it's morning sickness, only to discover that this can indeed be a side effect:

I have heard of it years ago from somebody who was receiving a larger than normal dosage. Morning sickness is person dependent as some women are very sensitive to it and others have no difficulty with it. I suspect if you check, you will find somebody in your family has had issues with it.

My levels are typical of first trimester pregnancy levels and I never experienced this. I take estradiol parenterally. The reported effects in transsexual women may be when oral non bio-identical estrogens were taken. Rarely, have I heard of this in women taking bio-identical estradiol. I took, in the past, high doses of oral estradiol and also didn't experience this. Progesterone, in higher doses (and at high concentrations), though has been associated with these side-effects because it slows down the gastric transit time.

Interestingly, I also came across this...

Br J Obstet Gynaecol. 1990 Oct;97(10):917-21.

"Increased levels of oestradiol may be beneficial, not only for their anti-depressant effect, but also in their effect upon bone density. The latter is greater after implant therapy than after oral therapy and is related to the greater plasma oestradiol levels achieved in women with implants. None of the 38 patients complained of alleged symptoms 01 oestrogen excess, such as breast tenderness, fluid retention or nausea."

"The mean serum oestradiol level of the 1388 women attending the clinic in 1988 was 767 pmol/l (range 78-2925 pmol/l), 66% had serum oestradiol levels <1000 pmol/l and 3% (38 women) had levels >1750 pmol/l (Fig 1)."

When I started taking a transitioning dose of E I noticed some intermittent nausea that came and went until my body adjusted to the higher dose. Give your body some time to adjust and see if it stops, and/or discuss it with your doctor.

In this study,

JAMA. 2009 August 19; 302(7): 774–780.

Only those taking the higher (VERY HIGH) oral dose of bio-identical estradiol occasionally experienced nausea/vomiting. 5 out of 32 (16%).

The TRANSSEXUAL PHENOMENON Harry Benjamin, M.D. 1966

"Diethyl Stilbestrol is the cheapest, but has the most frequent side effects in the form of nausea and gastrointestinal upsets. Better borne and rarely causing nausea is ethinyl estradiol in the form of Schering's Estinyl."

Ethinyl estradiol doses, at that time, were VERY high.

Ooooh, my sister had horrendous morning sickness, to the point of being hospitalised and on a drip.

Thanks Kay and Eva. 16% is quite a high figure really.

Well, I have to scale back anyway soon ahead of facial surgery. Going to be switching to gel for a while so I don't go 'cold turkey' but that's another story.

My levels were at pregnancy levels last month and I was definitely dealing with nausea off and on though I believe I was better off in the mornings than any other point in the day. They cut my dosage by 33 percent and I have little issues now. Hope they get your issue figured out quickly. It sucks to be nauseous.  Good luck hon.

God, yes.  I had bad morning sickness my first month on hrt.  It was horrible.

I've had morning sickness too.  It's all good now but given that I knew my HRT was working.  Hugs Shannon
