After trying to get an appointment through therapists in order to get a recommendation letter to bring to a PCP in order to start HRT, I've kind of given up as I can't exactly pay out of pocket for therapist visits which would cost me an arm and a leg as I don't exactly have the funds to do so and have to rely on city buses for transportation. Would anyone happen to know of any informed consent primary care physicians in the Jacksonville, Florida area that accept medicaid?

Welcome to Susan's Place. I am not familiar with the area but two places you should check would be Planed Parenthood and your local LGBT center. Local resources can often be found by googling "jacksonville LGBT". You can also do the same for therapist and some may take medicaid or adjust their rates according to your income.

Since you are on a tight budget, I suggest looking for a free trans group meeting. There you can network with people who may be in the same situation.
In my experience, there are very few Primary Care Physicians who will prescribe HRT from informed consent alone. It may be more expedient to find a low cost therapist who will give you the hormone letter with just a few counseling sessions. Look for a therapist who advertises a sliding scale fee. Usually less for those on a tight budget.
