I'm nonbinary transfeminine. Its caused me a lot of problems even figuring myself out, it took me and extra 15 years because initially I got chased out of trans forums with people yelling ->-bleeped-<- at me. So yeah, 15 years two kids, an ended marriage and one hell of a broken heart. I am 75/25 genderwise. I've been on hrt close to a year now, and I have to start asking myself the surgery questions.
I know that I need an orchiectomy. Ideally what I really want is to otherwise be both, but most of my research has told me that is impossible. In accouple rare mentions people have claimed to have found a doctor that can actually do that, or a doctor that has some kind of hermaphrodite option on grs. I figure the most likely realistic option is just let the hrt do its thing, socially transition and get an orchie and a labiaplasty, but I'd need a good surgeon that actually does that sort of thing and better advice on what would even be possible procedurewise .
As a side note, if anyone knows an excellent doctor near Maryland please say so.
Thanks for any and all help.

You need to spend time in the transtioning (https://www.susans.org/forums/index.php/board,339.0.html) section of the forum as both subjects are covered there. The problem with suggesting a surgeon is it depends on what you are after in a surgery. There are several different procedures with different goals in mind. In addition, cost and wait time are factors that determine the selection of a surgeon. One word of warning, you shouldn't have an orchi unless you consult with the surgeon doing the GCS. The reason is incorrect incision placement might make some of the tissue unusable meaning grafts or more invasive procedures to make do with the available skin. The really good surgeons will required some travel but you could stay on the east coast and access a great surgeon.

I am aware that I must get a GCS savy surgeon to do the Orchie. And I have been researching a lot, here and elsewhere. But the generic boiler plate information that is readily available is not what I need. I am ideally looking for comments from people that are on a non-standard path similar to my own. Currently it is looking like my GF's Thailand doctor might be the best bet.

Jade, fwiw I'd once have been glad to have both sets of genitalia, afaik nobody can do that for you. I asked my surgeon about keeping one testicle inserted as part of the surgery and got a firm no on the grounds that there would be no support structures.
Since then, however that desire has faded and as I've allowed for the possibility of GCS (beyond just possible now, it's 3 weeks away), my genital dysphoria has increased. So today if someone offered the hermaphroditic option I would pass on it. I'm still using my <shenis> because sexuality is part of what makes me feel good in my body and also because I don't want that skin shrinking, so as to have all the material possible for the procedure. And I'm still looking forward to having the right anatomy for my brain and spirit.
Good luck with the path you choose.
