Title: SA Gender Marker Stat Dec

Do you need a JP with you when the doctor signs it? If so, how do you manage that? I can't imagine my doctor leaving the office and the only JP I know lives interstate.

All of the medics that I'm aware of have organised someone to sign them. You leave that part of the form with them and they post it to you later.

It depends who is filling it out for you, the person who is doing mine said she would walk down to BD&M with me next session then it can all be signed and submitted in on hit.

Thanks, I'll see if I can get my doctor to get it done and give it back to me. Currently living interstate so will try throwing in this state's (ACT) form if he can't.
