Title: Short term disability question

I'm hoping someone will have had a similar experience anc can shed some light on this for me. I am ftm and have been on testosterone for 8 years. I've been with the company I'm employed by for 6 years so nobody knows that I'm transgender, and I really want to keep it that way. I work very closely to my HR department and know these people on a personal level. I'm going to be having a hysterectomy in the near future and will receive short term disability for the time that I'm out. I will have to submit a form from my doctor to my hr dept. in order to be approved for short term disability. In your experience have your doctors used different terminology to try to help disguise what type of surgery your getting done? Or how do they go about this?

Can your doctor just say it's for general surgery on the work hr form and then specifiy (as needed) for the disability form?   And then if you can send the disability form on your own rather than go through hr again.
