I am guessing that this is a normal response to Estradiol for MTF, but my nipples are always hard. I suppose it is a sign of breast growth, but I'm not sure.

Yep.... hard and erect nipples in the early stages of your HRT regimen are normal... then a few weeks or so later they get really sensitive, and can be painful if bumped...
... then soon enough there will be lumps under each nipple (breast buds) ... all of that means that HRT is working causing your breasts to grow!!!! :) ;)

Agreed, watch how close you are too doors when you open them. Also if you wear shirts with a breast pocket, be careful what you put there. I put my id there and scraped the skin once (not twice) and soon it will push the pocket forward. This is especially true is you are not out to everyone yet.
