Title: What do I need to do before seeing the endo?

Please forgive me if this question has been answered before. I went through old topics first but gave up when I wasn't finding what I wanted.
I'm planning to start HRT early next year, and I'll be seeing an endo outside of my health plan and paying out of pocket. In your experience, do I need to do anything other than make an appointment and bring my T letter? Can I get my first T shot on the first visit, or will I need to get blood tests first? Will I need to arrange a physical or a GYN visit before I see the endo, or anything like that?
I plan to ask my own endo these questions once I get my T referral, but I want to start T right on schedule and allow plenty of time for whatever extra appointments I might need. I might start HRT as soon as two months from now, and I want to have all my ducks in a row. And no snags, if that's possible.
