Unfortunately, the usual warning that you see on drugs are accurate, "Results may vary." There's a lot of variable factors that affect how much the hormones/anti-androgens can do. For some people, they see their hair completely disappear, while others hardly notice any change, and then there's everyone else in between. You should never get your hopes up that HRT will affect everything to the full extent.
Also, 13 months isn't an extremely long time to be on HRT yet, so more results may come later. As far as whether you should stay with the current dose or go back, I'd say just make sure you're within a safe dosage to get the desired results. If you're not seeing results by the increase then definitely go back down as it's unsafe and not beneficial to go beyond HRT's maximum effective dosages.

As Sarah said results do vary. I've heard some after starting HRT have no body hair reduction
and some have almost total body hair reduction. My chest and thighs are almost totally
hairless but I still have to shave my lower legs. You could always do laser which seems
to be very effective on body hair.
As far as orgasms I found that I have to be in the right mood, have had a good day,
be thinking happy thoughts... it's no more instant on and off anytime I want it to
happen. At first that was strange... it was like it felt good but I could not get there.
So I started to enjoy the process and not care about the end result and now I find
that I'm content without the actual climax. Overall I think orgasms become more in
line with a typical female response. And when it does happen... it lasts a lot longer
and words simply do not describe how much more satisfying they are.

Results vary, but some hair you will continue to retain after hrt for thirty years: face, groin (sometimes extending as far as the navel. Nor unusual for a lot of women to have that as well) and possibly lower back. If you've been shaving or waxing that will tend to build follicle and hair width. Try laser or elektro if you want rid of it.
And 13 months is very little time on HRT, and results do vary. I should have thought with a T reading of 0 the endo would have dropped you to a maintenace dose. BTW, everybody has T at some level or another. An ability to orgasm is tied to it evidently. Post-op women often take small T doses for health and sexual-response.
