are they expensive? what kind of hospitals do it? can i get one instead of going on anti androgens? do you need approval from your therapist to get one? do they remove the vas deferens along with the teste? how long is the procedure, and is there much pain afterward?

Depending on the clinic anywhere from ~$2,000 and up

Clinics and hospitals all do them, they are not very specialized surgeries.

Possibly. Discuss this with your doctor/surgeon/therapist

Yes. Possibly two letters depending on your location.
do they remove the vas deferens along with the teste?
Yes. In anticipation of GRS, most surgeons will remove much tissue from inside the scrotum. You can optionally have the scrotum removed also. Though that is used as possible tissue for GRS.
how long is the procedure,
About an hour.
and is there much pain afterward?
Not really. There is some discomfort but not that much pain as I recall.
