I noticed that there are some who have more natural looking breasts than others, what's the secret to have beautiful natural breasts with cleavage and all?

Do your homework, find a good surgeon.
Get a good bra (makes a huge difference).

I think if the implants are under you chest muscles rather than ontop they tend to look more natural.

Alternatively grow your own.

Cleavage usually takes a narrow ribcage. You can have huge implants and still not have cleavage if they're far apart.
Shop around, if possible try to see results up close and personal of different surgeons. I think the best bet is to go to a surgeon who has experience doing breasts on transsexual women, that way they can take into account the ribs and chest wall and everything.

Very enlightening! Does a surgery exist where they can move the both nipples more in the middle to get the two boobs to stick closer to have the cleavage?

The biggest factor on how natural breast implants look is how much you have to work with before you start. Going from an a cup to a DD is going to look fake no matter what. Going from a C cup to a D cup however looks entirely natural.

Unfortunately the ones who need implants the most, have the least to work with. Some surgeons will use temporary inflatable ones. This allows you to slowly increase the volume as your body adapts. This allows the skin and chest to adjust to the new size before moving up to the next size as you would naturally. Eventually when you reach the desired size, they are removed and replaced with normal implants.


As for moving the areola and nipple. If you move it, you will end up with scars at best. Some genetic girls have them quite widely spaced as well. The problem we have is implants will exagerate this. Some surgeons counter by placing the implant further apart.  This allows proper nipple placement, but you end up with the implant nearly hanging off the side of your chest and absolutely no cleavage at all.


It's a tough situation no matter how you go. The nest bet is wait and see what you end up with and try to be happy with it and maybe an enhancer (pushups, silicone or foam inserts and padded bras will help a lot) rather than surgery.

I think they look rather good, when I let them loose they sit near to what you see right now, and if I wear a push up bra, they can be squished togheter..  very natural for saline

Thats very interesting, I actually had saline implants, but they didn't always feel right, then getting the right bra to fit, never felt comfy. Never had a proper cleavage.
Then when I had them resized, I went bigger and went for silicone implants, they feel more natural, gosh they are big lol but with a support push up bra it gives me a really nice cleavage, it pushes them up and keeps them together, I love my girls, very fashionable in low neck dresses and tops, my BF adores them

Everything I'm reading lately seems to suggest silicone over saline for a natural look. Like most of us with this build, my main concern is that they not only look natural but that they suit the setting they will be resting in, natural in their spacing and so forth. Which I why I tend to think they need to be at least D's to accomplish that.

I don't have a narrow rib cage.  I used to have the 6K or 7K.  I now have gummy bears and I do have nice cleavage.

I love the gummy bears name it really suits them.  In the UK we call these sweets (candies) jelly babies which would also suit boobs rather well I think.

I have noticed from a few friends who have had BA's over the years that silicone wins over saline for the feeling of naturalness.   ;)

One friend is considering having her silicones replaced with larger one's.  Its not that she want to look like a porn star.  Her boobs look great in a bra and give cleavage.  The problem is that whe she showed me them au natural that they look too small.  I think it is important to get implants that are wide enough in the base to match the wider ribcage that a lot of mtf ladies must live with.

When I have mine done I think I will go for gummy bears.  I use prosthesis at present and worked with a (38) large A, small B cup for years.  I always felt I would be uncomfortable with larger ones and that people might stare at my boobs. I recently upped my size to a large C/small D and having got used to it feel it fits my body better.  I would certainly go for implants of this size

"Gummy" in this case does not mean sticky or chewy.
It comes from the German "Gummi" (rubber).
Gummibären are a type of Weingummis, a generic term for sweets made with gelatin to be rubbery.

I have saline implants and they look great if I do say so myself. I found a great local ps that does alot of breast augmentations every year. I was nervous about not having a doctor that specializes in transsexual breast augmentations until I meet with my doctor. He told me he had done about 10 ts girls before and that as long as you have at least an A-cup from hormones that it's no different then the surgery for a natal female with small breast. I had developed a B-cup from hormones. I have wide shoulders and I wanted a very large Anna Nicole/ Gia Darling look so the doctor used 800cc saline implants that he over filled to 965cc's, the implants are mod-plus profile which means that they are wider then some implants which helps to bridge the gap ts girls often get after breast augmentation. I'm one year post BA and I wear a 38DDD. I'll try to take a pic of me in a bra tomorrow so I can post it. I have great cleavage and a very small space between my breast. I chose saline implants because I wanted to have the least about of scarring possible. The doctor made a small ( 1 inch) incision hidden in my arm pit's to insert the implants. I have no scars on my breast! I highly recommend trying to find a doc that will go in through the belly button or arm pit. HTH!

That's great to hear you ladies are pleased with your saline implants, its always good to hear different points of view.  I know that saline are considered safer.  One friend had the expandable implant types.  It was funny as every time she visited her boobs were a little bit bigger - looked great when finished though. She developed great boobs having developed very little on hormones. 

I had heard of the ones with the wider base as that's what I need.  The trouble with local plastic surgeons here in Scotland is that they have little or no experience of TS ladies.  I am sure many thing that as long as we have bigger boobs after implants that we will be satisfied - but the've got to look right!  :)

The scarring issue is an important factor I would think.

Not sure what the pic policy here is but I'd like to see the results because I'm tempted to the larger sizes too (though maybe not to that level...not sure yet)

There is a small fill port they have access to to insert more and more saline. They are not meant to be permanent.

Expandable/temporary implants are used for two reasons.

The first is they can be used to get a better idea of the implant sizes needed, they can then order the exact fill size needed. Helpful when one breast is radically different than the other.

The other use is that you cannot put huge implants on a small chest without looking like a boulder attached. You put these in, and allow the body to adapt, then add more, then repeat. When you reach the size you want, they order implants based on how much they inserted into the implants.

As you had said yours were saline and were good I wondered if they were sub muscular.  All is revealed (or not in this case).  You certainly look full bosomed in your picture. Glad to hear they are a success.

I had gel "Gummy Bear" implants,  I was about a B before.  Now a big C or small 38D.  The implants were 400ml, the doctor recommended going with that size and it turned out very nice.  You can not feel the gel implant.  I do have a little scaring at the underside where the implants went in  Gel implants require a larger opening.  For my frame which is not small, dispite positive thinking, the implants are just about right that is proportional for my size.

I had breast augmentation myself when I was 18. However, I am not happy with the results anymore due to the 'bottoming out' effect which means my implants have moved to the sides and they look flattened because they are restricted under the pectoralis muscle and they are simply not big enough plus I don't like the gap I have. So I had a consultation yesterday with a good plastic surgeon who has a lot of experience with ts women. I have seen his work in other people I met through life. He told me that to get the natural looking breasts in a slim person, average size (380-400 cc) moderate profile smooth silicone implants on top of the muscle would be needed, tissues do not grow onto the smooth implants so they can have that natural movement of a breast and be soft but you have to massage them for the rest of your life to avoid capsular contracture which is like a %30 risk, however the moderate profile gives a wider cup size so it helps with having breasts that sit closer and the size I mentioned should get a ts woman to a C-D cup. I hope this helps. Whatever you do, stay away from a surgeon who will do under the muscle placement, it's a complex surgery if you need a revision in the future plus it damages the muscle and you'll get more scars trying to remove previous implants and it's painful. Most people can go on top of the muscle if they choose the right size for them, the only downside with that is risk of capsular contracture and sagging, but they can be avoided by regular massaging and wearing a good supportive bra.  :angel:
